Clinical Observation Of Qizhen Jiangtang Recipe On Early Diabetic Kidney Disease And Its Effect On Serum Inflammatory Factors IL-6,TLR4 And NF-κB

Objective: This study by observing the clinical efficacy of Qizhen Jiangtang Recipe in the treatment of early diabetic kidney disease(DKD)with Qi-Yin deficiency syndrome and the effect on serum IL-6,TLR4 and NF-кB levels in patients,preliminarily discussion the mechanism of Qizhen Jiangtang Recipe in preventing and delaying the progression of renal function in patients with early DKD,so as to provide new therapeutic ideas for the prevention and treatment of DKD in TCM.Methods: Fifty patients with early DKD with Qi-Yin deficiency syndrome were divided into two groups according to random number table,with 25 cases each of the control group and the treatment group.Both groups were treated with basic Western medicine,the control group was given Irbesartan tablets and the treatment group was given Irbesartan tablets in combination with Qizhen Jiangtang Recipe,course of treatment for 12 weeks.observed the changes of TCM syndrome score,FPG,Hb A1 c,TG,TC,LDL,HDL,Scr,BUN,UACR,and serum levels of IL-6,TLR4,NF-κB in 2groups before and after treatment.Explore Qizhen Jiangtang Recipe clinical intervention therapy in patients with early DKD.Results:1.Comparison of FPG and Hb A1 c indexes: After treatment,the FPG and Hb A1 c of the two observation groups were significantly lower than those before treatment(P < 0.01),but the reduction of the above blood glucose indexes in the treatment group was more significant than that in the control group(P < 0.05).2.Comparison of renal function indexes: After 12 weeks of treatment,Scr,UACR and BUN were significantly lower in both groups than before treatment(P < 0.01),but Scr,UACR and BUN were significantly lower in the treatment group than in the control group(P < 0.05).3.Comparison of lipid indexes: Comparing the two groups before and after treatment,TC and TG indexes decreased in the treatment group after treatment compared with those before treatment(P < 0.05),but LDL did not differ compared with those before treatment(P > 0.05);TC,TG and LDL did not differ significantly in the control group after treatment compared with those before treatment(P > 0.05).There was no significant difference between the two groups(P > 0.05).4.Comparison of inflammatory indexes: After treatment,serum IL-6,TLR4,and NF-κB levels were significantly lower in both groups than before treatment(P < 0.01),but the reduction of IL-6,TLR4,and NF-κB in the treatment group was better than that in the control group(P < 0.05).5.Comparison of TCM syndrome scores and clinical efficacy:: the total effective rate of patients in both groups after treatment was 79.2% in the control group and 95.7% in the treatment group,with statistically significant differences(P < 0.05);the TCM symptoms scores of patients in both groups decreased after treatment compared with those before treatment(P < 0.01),and the efficacy of the treatment group was better than that of the control group(P < 0.05).6.Safety analysis: no adverse reactions were observed in both groups throughout the trial.Before and after treatment,ALT and AST were compared between the two observation groups showing no statistically significant differences(P > 0.05);but Scr was lower in both groups than before treatment(P < 0.01).Conclusion: Qizhen Jiangtang Recipe has obvious clinical efficacy in treating patients with early DKD Qi-Yin deficiency syndrome.Compared with the control group,it was able to significantly improve patients’ clinical symptoms,correct the disorder of glucose and lipid metabolism,reduce the pathological indexes of renal function,and down-regulate the high expression of serum IL-6,TLR4,and NF-κB inflammatory factors in patients,reduce the inflammatory response.
