Colonoscopic Analysis Of 454 Elderly Cases Who Asked Health Examination

Objective:To retrospectively analyze the prevalence of colorectal diseases in the population aged>60 years old undergoing health examination in Zhangzhou region of Fujian and explore the significance of colonoscopy as a physical examination item for the elderly.Methods:The disease type,gender,patient age,time,colonoscopic findings and other clinical data of 454 elderly cases subjects voluntarily receiving colonoscopy in the Affiliated Southeast Hospital of Xiamen University from January 2013 to December 2017 were collected and made statistical analysis.Results:A total of 454 cases were included in the analysis.The results of colonoscopy in 179 cases(39.43%)showed no obvious abnormalities,275 cases of colorectal lesions,the overall detection rate was 60.57%.There were 230 cases of colorectal polyps(50.66%),27 cases of colorectal cancer(5.95%),23 cases of colitis(5.07%),9 cases of colorectal diverticulum(9 cases)1.98%),4 cases of colorectal lipoma(0.88%),3 cases of large intestine parasites(0.66%),2 cases of large intestine telangiectasia(0.44%)and 1 case of colorectal neuroendocrine tumors(0.22%).Among them,the detection rate of colorectal polyps in males was significantly higher than that in females(X2= 18.04,P = 0.00);the number of polyps whose sizes are less than 0.5cm was the most common,the polyps were mostly multiple,polyps were most detected in the rectum and sigmoid colon.In addition,the number of colorectal adenomatous polyps was 63 cases,accounting for 13.88%of the total number of cases,accounting for 27.39%of the total number of polyps detected.The detection rate of males was significantly higher than that of females(X2= 4.34,P = 0.04).Most of the adenomatous polyps are solitary polyps,accounting for 88.89%(56/63)of the total number of adenomatous polyps.The proportion of patients with adenomatous polyps larger than 0.5cm was 47.66%(61/128)and 5.58%(11/197),respectively.The difference between the two groups was statistically significant(X2= 79.64,P = 0.00).Large intestinal intraepithelial neoplasia detected a number of 30 cases,accounting for 6.61%of the total number of patients.The detection rate of males was significantly higher than that of females[9.26%(25/270),2.72%(5/184)respectively].There was significant difference between the sexes(X2 = 7.59,P = 0.01).The colorectal cancer was detected in 27 cases,accounting for 5.95%of the total number of physical examination,the detection rate of men and women were almost the same[5.93%(16/270),5.98%(11/184),respectively],There was no significant difference between the sexs(X2 = 0.00,P = 0.98).The highest detection rate was more than 75years old(18.92%,7/37).The difference between the age groups was statistically significant(X2 = 14.25,P = 0.00).Colorectal cancer detected most sites are the rectum,sigmoid colon,the total number of colorectal cancer 74.07%(20/27).Conclusion:Greater than or equal to 60-year-old elderly examination population colorectal lesions overall detection rate is higher.The types of diseases detected are more.Colorectal polyps is the highest rate of the statistical detection of diseases,including the detection rate of adenomatous polyps and intraepithelial neoplasia are also high,indicating that colonoscopy is an effective screening method for colorectal precancerous lesions in the elderly,for reducing colorectal cancer The incidence of significant.Colorectal cancer is the second highest rate of detection of disease,indicating that more than 60-year-old population is a high incidence of colorectal cancer.The colonoscopy as an elderly population screening is necessary.
