Objective:To explore upper gastrointestinal disease situation and the necessity of Gastroscopy health examination people,strengthen the awareness of Gastroscopy in health examination people and provide clinical evidence for enhancing the detection rate of early cancer.Methods:Through the questionnaire survey of 18613 staffs in TISCO Group,9885 subjects without contraindications were selected to give endoscope examination,who were informed by the phone and voluntarily signed informed consent before endoscope examination.This paper performed a retrospective analysis of 4647 subjects' gastroscopy results from the Endoscopy Center of TISCO General Hospital,from January 2014 to December 2016,and explored the relation of gender,age,and disease detection rate.Results:In the 4647 subjects of health examination,the age was range from 27 to 74,with an average of 47.48±5.82.873 cases were females(18.8%),and 3774 cases were males(81.2%),801 cases were in the youth group,under 39 year-old(17.2%),3717 cases were in the middle-aged group,between 40-59 year-old(80.0%),and 129 cases were in the elderly group,over 60 year-old(2.8%).And in terms of diseases types,465 cases were reflux esophagitis(10.0%),84 cases were Barrett esophagus(1.8%),3531 cases were chronic superficial gastritis(72.0%),243 cases were chronic atrophic gastritis(5.2%),673 cases were peptic ulcer(14.5%),498 cases were gastric polyps(10.7%),41 cases of uppergastrointestinal cancer(0.9%).There was not statistically significant between female group and male group in detection rate of Barrett esophagus and upper gastrointestinal cancer(P>0.05).However,there was statistically significant between female group and male group in the detection rate of reflux esophagitis,chronic non-atrophic gastritis,chronic atrophic gastritis,peptic ulcer and gastric polyp(P < 0.05).It was high that the male detection rate refiux esophagitis,chronic atrophic gastritis,peptic ulcer and upper gastrointestinal cancer,but it was inverse for the chronic non-atrophic gastritis,gastric polyp.There was not statistically significant among different groups based age on the detection rate of Barrett esophagus and peptic ulcer(P>0.05),However,there was statistically significant for chronic non-atrophic gastritis,reflux esophagitis,chronic atrophic gastritis,gastric polyps,upper gastrointestinal cancer(P<0.05).In middle-aged group it was main for reflux esophagitis,chronic atrophic gastritis,but in elderly group it was main for chronic non-atrophic gastritis,gastric polyps.There were 41 cases were upper gastrointestinal cancer(0.9%),8 cases were early carcinoma(19.5%),33 cases were advanced carcinoma(80.5%),there was no statistically significant for upper gastrointestinal cancer in different gender and different ages(P>0.05),Over 40 year-old people it was detected for early carcinoma.Conclusion:It has a high disease detection rate in gastroscopy of people over 40 years old for different upper gastrointestinal tract diseases and has gender differences in it.Especially,in this group,there was a considerable detection rate of upper gastrointestinal tract early cancer.Therefore,people over 40 year-old should attach importance to gastroscopy to reduce early carcinoma rate by improving detection rate.
