[OBJECTIVES]1.To investigate the incidence of depressive symptoms in the elderly in Chengdu community and to analyze the influencing factors.2.To explore the effect of integrated TCM and western nursing intervention on depressive symptoms and quality of life of the elderly.[METHODS]This topic includes two stages of research.The first stage: investigation study.Selected three representative communities in Chengdu,randomly selected 400 elderly people from those community which match the inclusion and exclusion criteria,all of them were investigated with self-designed General Condition Questionnaire and the Geriatric Depression Scale,analyze the data,summed up the incidences of depressive symptoms in the elderly population.The second stage: intervention study.Choose 60 of the elderly who have depressive symptoms,randomly divided into intervention group and control group,the intervention group using the integrated TCM and western nursing intervention,the control group Cognitive therapy to intervene.The quality of life was assessed by the Short Form 36 Health Survey questionnaire(SF-36)before intervention.GDS and SF-36 were used to evaluate the depressive symptoms and quality of life of the two groups after intervention.Evaluate the effect of integrated TCM and western nursing intervention.[RESULTS]1.Among the 399 elderly people,there were 145 elderly have depressive symptoms.The incidence of depressive symptoms was 36.4%.110(27.6%)with mild depressive symptoms and 35 with moderate or severe depressive symptoms(8.8%).The GDS score ranged from 0 to 30,with an average of(9.61 ± 6.92)?2.Univariate analysis and multivariate regression analysis showed that regular physical exercise,high self-evaluation and optimistic were the protective factors of depressive symptoms in the elderly.3.The remission rate of depressive symptoms in the intervention group was better than that in the control group(P <0.05).4.There were significant differences in the quality of life scores in the intervention group before and after intervene(P <0.05).5.There were significant differences in the scores of physiological function,emotional function,physical pain,energy and mental health of the elderly in the control group(P <0.05),but no significant differences in other dimensions(P > 0.05).6.There were significant differences in the scores of physiological function,social function,energy,mental health and general health status of the two groups after the intervention(P <0.05).[CONCLUSIONS]1.The incidence of depressive symptoms in the elderly in Chengdu was 36.4%,which should be paid attention to by community health workers.2.The remission rate of depressive symptoms were higher in these elderly who have poor marital status,no fixed source of income,low income,irregular life,no hobbies,do not take the initiative to health,not exercise,long-term smoking,suffering from chronic diseases and pessimistic.3.The occurrence of negative events can increase the probability of depressive symptoms in the elderly.4.Regular exercise,high satisfaction with their lifes in there own social and families and optimism are the protection factors of depressive symptoms in the elderly.5.The integrated TCM and western nursing intervention has a significant improvement effect on the depressive symptoms and quality of life of the elderly in the community.6.The integrated TCM and western nursing intervention treatment program is superior to simple cognitive therapy in the improvement of depressive symptoms and quality of life of the elderly in the community.
