Objective1.Use the Theraaeutic Intervention Scoring System(TISS)to calculate the ICU nursing workload and assess the nursing workload of each nurse who has the ability to work independently and participates in direct care to understand the workload of the clinical nurse.2.To explore the changing trend of ICU nursing workload,to provide the theoretical basis and data for flexible management of nurse managers,to rationally allocate human resources,and to reduce the workload of clinical nurses.3.Through nursing job satisfaction scale,we can understand the job satisfaction of ICU nurses and improve them from the aspect of lower nurses' satisfaction so as to improve the overall level of job satisfaction of clinical nurses.Methods1.Research object: This study uses a convenient sampling method to extract 42 people from all nurses in the ICU Hospital of a class three grade a hospital in Henan Province,which conforms to the inclusion criteria,and selects the time of one week from January 2017 to June,and the 42 nurses with ICU participation in direct nursing and independent working ability are divided into each class.The TISS-76 score table was used for each patient to count the nursing work performed by the patient as a nurse's day of nursing work for 42 days,and 840 patients were assessed.2.Survey tools: the survey tools are divided into 3 scales.The first part is a questionnaire for the general demographic data and work situation of nursing staff.The general situation of nurses includes gender,age,working time,existing job title,the highest degree of education,and the average monthly income;and nurses' work conditions include Whether the current mode of duty is satisfactory,the nurse thinks the maximum working load time period(24 hour system),annual vacation rest,overtime(hour / month),overtime pay for economic compensation or supplement,the questionnaire is used to collect the general demographic data of nurses;the second part is that the TISS-76 scale is repaired by Keene.A total of 76 monitoring and treatment items were included,and each nursing operation was assigned 1~4 points respectively.According to the total score of each patient,the patient's condition was divided into 4 grades,grade I 0 ~9,grade II,10 ~19,grade III 20,~39,grade IV or 40,according to the TISS scale,the higher the score,the more serious the scores.They are not satisfied with the current working situation.3.Method of investigation: The ICU hospital center of a three grade a comprehensive hospital in Henan province was selected to connect with the leadership of the hospital nursing department and the central ICU ward,to obtain consent and assist in the issuance and recovery of the questionnaire.Through the survey of the general demographic data of the nurses in the center of ICU and the working conditions of the nursing staff,the nursing workload was analyzed and the satisfaction of the nurses was analyzed.A total of 42 questionnaires were distributed and 42 were recovered,with a recovery rate of 100%.From January 2017 to June,a week was taken at random.Each patient was assigned to each nurse using the TISS-76 scale to count the nursing program and the nursing workload on the day of the nurse.Every day the TISS scale of all the nurses in charge of the patient was added and taken as the nursing care of the day.Workload;continuous statistics for a week as the workload of the month,a total of 42 days,the nurse statistics the effective TISS scale of 840 patients.4.Statistical analysis: input all data into the database established by SPSS17.0,and use SPSS17.0 statistical software to evaluate the data after statistical processing.The scores of nurses' general data,weekly nursing workload and job satisfaction scale were described by mean,standard deviation or frequency and percentage,and the relationship between general data and satisfaction of statistical nurses was tested by parameter test(t test or variance analysis).Result1.From January to June 2017,a random sample of weekly ICU nurse workload was collected.The statistical results showed that during the week,the number of nursing work was the largest(527.50±33.57)points.The minimum nursing workload(446.170±41.45)on Sunday(P<0.001).2.The relationship between the general demographic data of nurses and the job satisfaction of nurses and nurses' job satisfaction showed that the nurses' job satisfaction decreased with the increase of nurses' age,professional titles and working years.3.From the scores of the nurses' job satisfaction scale,it was found that the overall level of ICU nurses' job satisfaction was moderate,and the scores of recognition and praise,interpersonal relationship,achievement and responsibility were relatively high in the 9 dimensions,and the scores of wages and subsidies were the lowest.4.In the correlation analysis between nurses' workload and nurses' degree of satisfaction,the total workload scores of nurses in 6 months were positively related to the dimension salary and subsidy and the professional characteristics.In January,the workload was positively related to the acceptance and praise,the salary and subsidies,the scheduling and the working conditions and the interpersonal relationship;in March,the workload and the workload of the nurses were positively related.There is a positive correlation between the balance dimension of the family and the work.Conelusions1.The results of this study show that the workload of ICU nurses is large,and it is necessary for the managers to arrange the nursing human resources reasonably according to the changing trend of the statistical workload.2.Compared with low year,low professional titles and low age nurses,high year,high professional titles and elderly nurses have low job satisfaction.The managers should improve the job satisfaction of senior nurses in order to play an important role in the work.3.The scores of nurses' job satisfaction scale showed that the average score of salary and subsidy was the lowest.Therefore,improving the salaries and welfare benefits of nurses is an important part of improving nurses' job satisfaction.4.There is a positive correlation between the total amount of work and the satisfaction of the nurses in six months,and the relationship between the two dimensions of the salary and the characteristics of the occupation itself.
