Evaluation Of Clinical Value Of 4 Kinds Of Test Scores In Patients With Acute Exacerbation Of Chronic Obstructive Pulmonary Disease

Objective:To explore the clinical value and significance of the chronic obstructive pulmonary disease(COPD)assessment test(CAT),general hospital anxiety / depression(HAD)mood test,pittsburgh sleep quality index(PSQI)score and daily living ability(Barthel index)in patients with acute exacerbation of COPD(AECOPD).Methods:A prospective observational study was conducted,AECOPD patients were admitted to the hospital from October 2017 to February 2018 were enrolled,and they all underwent CAT assessment test,HAD test,PSQI test,Barthel index test and lung function examination.The patients were assigned to ?,? and ? grade groups according to the lung function level,and the difference of forced expiratory end volume in 1 second(FEV1)and FEV1/forced vital capacity(FVC)ratio were compared among the three groups;the patients were also assigned into 2,3 and 4 grade groups according to CAT scores,HAD scores,PSQI scores and Barthel index scores,and the differences of length of stay in the hospital and hospitalization expenses were compared among the three groups.Spearman correlation analysis was used to analyze the correlations between the 4 kinds of score and lung function indexes.Results: Sixty patients with AECOPD were accepted and all of their clinical data were analyzed in the study.Their mean FEV1 was(0.52±0.19)L,FEV1/FVC was 0.47±0.13,CAT score was 27.74±5.74,HAD score was 13.92±2.16,PSQI score was12.27±2.16,and Barthel index score was 57.28±13.95.The correlation analyses showed:there were significant negative correlations between CAT score and FEV1,FEV1/FVC(r value was-0.684,-0.782,both p <0.05),there were negative correlations between HAD score and FEV1,FEV1/FVC(r value was-0.516,-0.542,both p <0.05),there were negative correlations between PSQI score and FEV1,FEV1/FVC(r value was-0.527,-0.671,both p <0.05),there were no correlations between Barthel index score and FEV1,FEV1/FVC(r value was 0.313,0.352,both p >0.05).With CAT score,HAD score and PSQI score getting higher ang higher,the length of stay in hostital(day)and hospitalization expenses(yuan)of patients with AECOPD were increased(2,3,4 grade group of CAT score: 6.65±1.32,8.97±2.82,13.21±4.86 and 6231.67±1139.16,8348.05±2419.37,11029.53±4038.27,all p <0.05;2,3,4 grade group of HAD score:6.93±1.67?13.65±2.58?16.94±5.17 and 6547.18±1047.59,8763.62±2195.59,11631.51±3879.24,all p <0.05;2,3,4 grade group of PSQI score: 6.93±1.67?13.65±2.58?16.94±5.17 and 6956.45±1213.78,8283.58±2619.29,12739.55±4163.18,all p <0.05).Conclusion: CAT,HAD and PSQI are reliable tools to measure the life quality of AECOPD patients and have certain relevance to the severity and prognosis of the disease.
