Study On The Rate Of Drug Treatment,Compliance Of Patients With Hypertension And Its Influencing Factors In Lianyungang Rural Areas

Objective:By use the Morisky adherence questionnaire and self-made demographic questionnaires to investigate the drug utilization and compliance of patients with hypertension in Lianyungang rural areas.To find the influence factors that effect compliance and the rate of drug treatment,So as to provide comprehensive evidences for the prevention and treatment.Methods:With multi-stage sampling,2098 hypertensive patients were selected from five towns in Lianyungang,organized them to the local hospitals for blood pressure measurement,physical examination and questionnaires,attained their fundamental information and drug utilization,evaluated the compliance of hypertensive patients.We used Epidata3.1 to establish a database and SPSS 16.0 statistical software was used for data analysis.Univariate?~2 test and binary logistic regression model were used to analyze all kinds of key factors.Results:(1)2098 patients were selected,including 913 men(43.5%)and 1185(56.5%)women,the average age is 55.72±8.96 years old.The rate of drug treatment and control of hypertension were 62.2%and 13.3%,The compliance of patients with hypertension is 27.8%.The highest rate of drug treatment and compliance of patients are Shiliu and Wenquan,the lowest are Wenquan and Shiliu.(2)The rate of drug treatment:male was 60.6%,female was 63.5%(P>0.05);35~45~,55~,65~,75 years old and above were 45.3%,57.1%,65.1%,64.8%,70.6%(P<0.05);in different education level were 61.9%,62.9%,61.1%,70.6%(P>0.05);underweight,normal,overweight and obese patients were 55.9%,56.9%,62.6%,67.4%(P<0.05);farmers and non farmers were 68.8%,61.1%(P<0.05);patients with different monthly income were 62.5%,62.4%,60.2%(P>0.05);in different marital status were 59.9%,77.5%(P<0.05);in different family history were 66.6%and 57.9%(P<0.05);in different illness history were 44.6%,80.4%,82.5%(P<0.05);in different blood pressure levels were 85.6%,53.4%,65.9%,65.5%(P<0.05).(3)The compliance of patients with antihypertensive therapy:male was 33.3%,female was 23.8%(P>0.05);35~,45~,55~,65~,75 years old and above were 4.5%,24.9%,25.5%,35.9%,30.9%(P<0.05);in different education level were 24.7%,27.0%,30.6%,42.7%(P<0.05);underweight,normal,overweight and obese patients were31.6%,29.5%,28.8%,24.6%(P>0.05);farmers and non farmers were 27.9%,24.9%(P>0.05);patients with different monthly income were 28.2%,22.6%,33.9%(P>0.05);in different marital status were 30.1%,16.1%(P<0.05);in different family history were 37.3%and 16.8%(P<0.05);in different illness history were 21.8%,29.4%,33.8%(P<0.05);in different blood pressure levels were 31.5%,27%,28%,24.3%(P>0.05).(4)The comparison of four items of Morisky adherence questionnaire shows that 815patients(62.4%)answer“yes”when they were asked“whether forget their medication”and only 355 patients answer“yes”when they were asked“the awareness is bad,whether stop using drugs”.Obviously,"forget medication"is the main reason of patients with poor compliance.(5)The influencing factors of drug treatment rate:age,profession(OR=1.459),marital status(OR=1.907),family history(OR=0.752),course of disease,body mass index(BMI),blood pressure grade.(6)The influencing factors of compliance of patients with antihypertensive therapy:sex,age,education level,marital status(OR=0.368),family history(OR=0.322),course of disease and purchase time.Conclusions:The results showed that the rate of drug treatment is relatively high,but the rate of compliance is low in rural area of Lianyungang.In order to improve the rate of drug treatment and medication compliance,patients should pay more attention to their illness,doctors should be more responsible,the government should increase investment,through multi angle efforts to keep their blood pressure at desired levels.
