Objective: To preliminarily explore the relationship between coronary heart disease and H-type hypertension in middle-aged men,to provide the basis for clinical diagnosis and treatment.Methods: 231 middle-aged men suspected of coronary heart disease hospitalized in the cardiovascular medicine department of Affiliated Hospital of North Sichuan Medical College and underwent coronary angiography(Coronary Angiography,CAG),and 50 healthy persons who underwent routine physical examination in our hospital physical examination center were selected as subjects from January 2016 to January 2017.According to WHO's definition of middle age,the enrolled age group was 40-65 years old.1.According to the results of CAG,all the patients suspected of coronary heart disease were divided into coronary heart disease group(150 cases)and non coronary heart disease group(81 cases),a total of 50 healthy persons were selected as control group,collecting and comparing the general clinical data between coronary heart disease group and non coronary heart disease group and control group.2.According to the level of Homocysteine(Hcy)and whether combining with hypertension,150 patients in the coronary heart disease group were divided into 4 subgroups: simple HHcy,common hypertension,H-type hypertension and both normal group.We compared the degree of coronary artery lesion(Gensini score)between the subgroups.3.According to the results of CAG,the group of coronary heart disease was divided into 2 subgroups of single-vessel disease and multi-vessel disease.we compared the Hcy level and proportion of H-type hypertension subgroup between the two subgroups.4.According to clinical symptoms and auxiliary examination,the group of coronary heart disease was divided into 2 subgroups of stable angina pectoris(SA)and acute coronary syndrome(ACS),we compared the Hcy level and proportion of H-type hypertension subgroup between the two subgroups.Results:1.The proportion of smoking,hypertension,H-type hypertension and Hcy level in CHD group were all higher than that in the non coronary heart disease group,the difference was statistically significant(P<0.05).The general data including serum lipid metabolism and diabetes,etc were no significant difference between CHD group and non coronary heart disease group,the difference was not statistically significant(P>0.05).The results of Logistic regression of two groups showed that smoking,hypertension,Hcy and H-type hypertension were independent risk factors for CHD in middle-aged men,and the difference was statistically significant(P<0.05).2.The proportion of smoking,body mass index(BMI),diabetes,triglyceride(TG),hypertension,Hcy level and H-type hypertension were all higher than that in the control group,the difference was statistically significant(P<0.05).The general data including age and cholesterol(TC),etc were no significant difference between CHD group and control group,the difference was not statistically significant(P>0.05).The results of Logistic regression of two groups showed that smoking,BMI,diabetes,TG,hypertension,Hcy and H-type hypertension were independent risk factors for CHD in middle-aged men,and the difference was statistically significant(P<0.05).3.The Gensini score in H-type hypertension subgroup(65.54±21.15)was higher than that in simple HHcy subgroup(39.48±23.36),common hypertension subgroup(31.91±18.99)and both normal subgroup(26.40±13.07),the difference was statistically significant(P < 0.05).4.The Hcy level(14.77±6.76?mol/L)in multi-vessel disease group was higher than that in single-vessel disease group(12.40±2.91?mol/L),the difference was statistically significant(P<0.05).The proportion of H-type hypertension in multi-vessel disease group(65.31%)was higher than that in single-vessel disease group(36.54%),and the difference was statistically significant(P<0.05).5.The Hcy level(15.36±5.83?mol/L)in ACS group was higher than that in SA group(12.85±5.75?mol/L),the difference was statistically significant(P<0.05).The proportion of H-type hypertension in ACS group(68.81%)was higher than that in SA group(19.51%),and the difference was statistically significant(P<0.05).Conclusion: H-type hypertension and Hcy are independent risk factors of CHD in middle-aged men,and have a positive correlation with the severity of coronary lesion.
