| Objective: To investigate the application value of serum total homocysteine levels in coronary atherosclerosis heart disease of middle aged persons,particularly in acute myocardial infarction.Methods: Dividing 221 middle aged persons (male 176 cases,female 45 cases)who was admitted to heart internal medicine department into three groups according to coronary artery angiography:1 group(acute myocardial infarction)51 cases;2 group (coronary atherosclerosis heart disease and except acute myocardial infarction)72 cases;3 group(free of coronary atherosclerosis heart disease)98 cases. According to the standard of World Health Organization the diagnosis of acute myocardial infarction required 2 or more of the following 3 criteria:lasting and severe chest pain that did not disappear in rest,characteristic changes on electrocardiography,and specific enzyme elevation. coronary atherosclerosis heart disease was defined as a stenosis of at least 50 percent of the vessel diameter in any of the main coronary arteries (the left main coronary artery or the left anterior descending coronary artery with its major diagonal branches, the right coronary artery, or the circumflex coronary artery with its major marginal branch). Depending on dominance, the posterior descending branches or posterior branches of left ventricular was included as part of right coronary artery or the circumflex coronary artery. All of the above cases were tested adopting Philip Digital Angiography machine(Allura9) during hospitalizing. This research wasn't including the coronary heart disease of general clinical diagnosis. Excluding standard: malignant tumor, psoriasis, hypothyroidism and using medicines which influence homocystein level , like methotrexate, carbamazepine, phenytoin, binitric oxide, 6-nitrogenuraciline and many kind of vitamins. Venous blood samples 3 milliliter were obtained after fasting 8 hours. Samples were centrifuged after 30 minutes, 0.4 milliliter serum was stored at -15℃. Measure serum homocysteine levels and analyse the datas with statistical methods.Results: The difference of sex, age, diabete and impaired renal function between groups showed no statistical significance(P>0.05). Geometric mean serum total homocysteine was the highest in 1 group, higher in 2 group, the difference was statistically significant(P<0.05).Conclusions: The elevation of serum total homocysteine levels of the middle aged persons in Fushun area which could contribute to the explation of geographical variation and deviation of the lifestyle is very common. the serum total homocysteine levels is closely related to coronary arterioclerosis heart disease in middle aged persons, particularly to acute myocardial infarction.
