Chronic hand eczema is a pruritus dermatosis caused by a variety of exogenous and endogenous factors.Its skin lesions are mainly erythema,scaly,infiltration and hyperkeratosis.Itching is the main symptom,and it can also cause a pain in the chapped skin after the activity.When it is corresponding to the "E Zhang Feng" of Chinese medicine,it is more consistent with its clinical features.At present,western medicine for the treatment of this disease is more difficult.and there is no satisfactory scheme.Topical glucocorticoid ointment is the main way of treatment.Although it can achieve good results at the early stage,the efficacy is gradually decreasing with the prolongation of the time of use,and the condition starts to relapse,and the adverse reactions also follow.This forces the patients to seek new treatment plan again.Traditional Chinese medicine has unique advantages in the treatment of chronic hand eczema.especially in the external treatment of traditional Chinese medicine.It is possible to choose suitable dosage forms based on the proportion of the prescriptions or according to the different population,the location of the disease and the stage of disease.In this study,Modified Wufuqushi ointment and Binghuangfule ointment were used to treat chronic hand eczema.The changes of skin lesions,pruritus,pain and other symptoms were observed before and after treatment,and the effects on the skin barrier function were observed.ObjectiveObserve the curative effect of Modified Wufuqushi ointment and Binghuangfule ointment,evaluate the effect of Modified Wufuqushi ointment better than Binghuangfule ointment,and provide data support for clinical application and popularization.To observe the effect of Modified Wufuqushi ointment on the function of local skin barrier,and to provide more objective basis for its curative effect.With a view to developing new medium ointments to reduce or replace the use of glucocorticoid ointments.MethodThe subjects were divided into the test group and the control group by the random control method.The experimental group was applied with Modified Wufuqushi ointment,and thecontrol group was applied with Binghuangfule ointment.The two groups were 2 times per day and 4 weeks were a course of treatment.The severity index of hand eczema,the itching and pain visual analogue scale and the adverse reactions during the treatment were recorded before treatment,2 weeks and 4 weeks after treatment.Skin disease quality of life index was evaluated before and after treatment,and skin moisture and oil content were detected to evaluate the improvement of patients’ quality of life and the effect of skin barrier function.ResultThe optimization of Modified Wufuqushi ointment,the total effective rate was 82.1%;the Binghuangfule ointment total effective rate was 71.4%.There is no significant difference in statistics between the two.However,there were significant differences in symptom improvement(HECSI),and the Modified Wufuqushi ointment was better than the Binghuangfule ointment.The two groups were improved before and after treatment,such as erythema,scaly,moss like changes,chapped and skin lesions.Especially in the area of skin lesion improvement,the Modified Wufuqushi ointment was superior to the Binghuangfule ointment,and there was a significant difference.In the improvement of skin moisture and oil,the two groups were improved after treatment,the Modified Wufuqushi ointment was superior to the Binghuangfule ointment,and there was a significant difference.ConclusionThe Modified Wufuqushi ointment can improve the symptoms of skin lesions,reduce the area of skin lesions,improve skin moisture and oil content,and restore skin barrier function.It is an effective drug to treat chronic eczema of the hand,and is suitable for clinical application and promotion.
