| Overview:Chronic eczema is a common disease in department of dermatology, it's lingering, refractory, and recurrent and brings a lot pain to the patient's life. Chinese medicine external treatment compared to western medicine external creams are inexpensive, non-toxic effects, and effective. My tutor associate professor Hua hua, took the folk recipe of coptis, Perillae folium, cassia bark incorporated with damp toxin clean ointment as the external treatment of chronic eczema in clinical application, and the curative effect reflected by patients was obvious. This thesis includes in literature review and clinical research two parts. There are pieces two literature reviews.The first one named"the traditional Chinese Medicine Research Survey of eczema"is about the general situation of eczema research in traditional Chinese medicine, includes the historical names of eczema, recognition of the pathogenesis of eczema, internal and topical treatment. It contains various records of disease names similar to eczema of ancient Chinese literatures, and etiology and pathogenesis discussion about eczema, recognition of the pathogenesis of eczema, therapeutic principle and medication of eczema of ancient physicians. The new research progress of eczema within ten years is also involved, contained several famous elder TCM doctors recognize of etiology and pathogenesis of eczema, recognition of the pathogenesis of eczema, therapeutic principle and medication of eczema. The second one named"the modern medicine research development on eczema"is about the research progress of modern medicine over pathogenesis and treatment of eczema in the past decade.Objective:This subject aims to address the clinical effect of modified eczymatous ointment (coptis, Perillae folium, cassia bark incorporated with damp toxin clean ointment) of chronic eczema, to provide advanced clinical guidance.Methods:72 cases of patients were selected by inclusion criteria exclusion criteria, from patients of department of dermatology Guang An Men hospital. Informed consents were signed. Then they were randomly divided into two control groups,36 cases for each. Control groupl:damp toxin clean ointment + powder of coptis, Perillae folium and cassia bark was applied on the left side of body, damp toxin clean ointment was applied on the right side. Control group 2:damp toxin clean ointment + powder of coptis, Perillae folium and cassia bark was applied on the left side; powder of coptis, Perillae folium and cassia bark + cosmolin was applied on the right side. We used the EASI scoring method to calculate the degree of itching, redness, pimples or small blisters, erosion, exudation and the other symptoms and made out the scores. Take the method of parallel group and self control we continued the treatment for 4 weeks. Then we used the EASI scoring method to score the degree of itching, redness, pimples or small blisters, erosion, exudation and the other symptoms after the treatment to observe the curative effect. By comparing the clinical symptoms and changes of EASI sores before and after the treatment, the efficacy determine standard = [(pro-treatment points-post-treatment points)÷pro-treatment points]×100%. Clinical cure:All skin lesions subside, itching disappeared and only left hyperpigmentation, efficacy rate:100%; markedly:most skin lesions subside, itching significantly reduced, efficacy rate:70% or more; effective:some lesions subside, itching eased, efficacy rate:30% or more; invalid:no significant lesions subsided, itching unreduced or clinical symptoms deteriorated, efficacy rate of less than 30%. The statistical analysis was by SPSS18.0 software, statistical methods were the t test and rank sum test.Results:The modified eczymatous ointment treatment of chronic eczema, cure rate:11.1%, markedly effective rate 27.8%, efficiency 47.2%,13.9% inefficiency, the total effective rate:86.1%; The damp toxin clean ointment group cure rate:2.9%, markedly effective rate 5.7%, efficiency 74.3%,17.1% inefficiency, the total effective rate:82.9%.The curative effect of modified eczymatous ointment group was better than damp toxin clean ointment group (P=0.028). The powder of coptis, Perillae folium and cassia bark + cosmolin group, cure rate:2.8%, markedly effective rate 11.1%, efficiency 69.4%,16.7% inefficiency, the total effective rate:83.3%, the damp toxin clean ointment group and the powder of coptis, Perillae folium and cassia bark + cosmolin group had no therapeutic differences (P=0.668)... |
