Research On The Rule Of Prescription For The Treatment Of Eczema By Chinese Medicine Based On Data Mining

Objective:Eczema is a common allergic dermatosis caused by a variety of internal and external factors.Based on the literature of eczema treatment with traditional Chinese medicine,this study investigated the rules of eczema treatment with traditional Chinese medicine in order to provide the information support for clinical medication,tried to discover new medicine combination and prescription,and explore new guidelines for eczema treatment with traditional Chinese medicine.Method:After the collection of all eczema prescriptions in six traditional Chinese medicine monographs:"Effective tradtional Chinese Medicine cosmetology prescription","Zhang Zhili skin disease clinical prescriptions","Ancient and current experts prescriptions of skin Disease","Skin diseases-great traditional Chinese Medical Classic Case-illness and symptoms","The classical cases of dermatology","External traditional Chinese medicine prescription of skin diseases",we created a database using the software of "Traditional Chinese Medicine Inheritance Assistant System(V2.5)".By applying the association rule Apriori algorithm and entropy clustering as the main data calculation method,we extracted and analyzed all the eczema prescriptions' frequency of use,drug combination,association rules,core drug combinations,and we also developed new prescriptions.At the same time,we use this method to dig and analyze the prescription containing digitalis(the most frequently used drug).We combined the basic theory of traditional Chinese medicine with experts' clinical experience,summarized the key treatment rules,drug combinations and core prescriptions at different stages of the disease.Meanwhile,we intensively analyzed the most common drug of fresh Rehmannia.In addition,on the basis of data mining,we get several drug combinations,and select a typical " Fresh white skin-Fructus Kochiae" drug combination with high frequency and correlation degree,relying on the traditional Chinese Medicine system Pharmacology Analysis Platform(TCMSP),through constructing the prediction target network,PPI network related eczema,Potential target Network of" Fresh white skin-Fructus Kochiae" drug combination to systematically analyze the mechanism of"Fresh white skin-Fructus Kochiae" drug combination in treating eczema.Results:(1)We collected 206 internal prescriptions,including 229 drugs.The top three were fresh Rehmannia(93 times),Fresh white skin(90 times),Coix seed(71 times).The most often usedcombinations is”Scutellaria-fresh Rehmannia "(44 times)," fresh Rehmannia-paeoniae "(43 times)," Fresh white skin-fresh Rehmannia "(41 times)," Fresh white skin-Coix seed "(41 times).This indicated that clearing heat,cooling blood and removing dampness is the main principle for acute and subacute eczema treatment.We summarized all drugs properties and characters,of which cold drugs accounted for 62.1%,bitter drugsaccounted for 35.2%.Drugs into the liver accounted for 17.5%.Most eczema is due to blood heat and liver functions to maintain blood,so the treatment of eczema is mainly the cold drug into the liver.According to the association rules and cluster analysis of the drugs,we deduced 3-4 core drugs combinations;9 new prescriptions and 18 pairs.Combining the modern pharmacological research and the experience of clinicians,it was concluded that 5 prescriptions have certain clinical value"Schizonepeta_Plantago_Alisma_Cicada slough"has the effect of stopping dampness and itching.It is concluded that the main function of this prescription is to cure inflammation and swelling,reduce exudation,which is consistent with the pathological mechanism of eczema.hPolygonum multiflorum-Mulberry leaves-Cicada slough-snake slough "has the effect of nourishing blood,moistening dryness,stopping itching,and it can be used for chronic eczema of blood deficiency."Scutellaria-Salvia_Poria cocos ?Gardenia"cutellaria-Salvia clearing heat and activating blood and removing dampness.It can be used in the beginning of eczema and chronic eczema of blood stasis blocker."Astragalus-rhizoma atractylodis-cassia twig-Polyporus umbellatus-Poria cocos"has the effect of keeping breath and dispelling dampness,suitable for chronic eczema of dampness resistance.It can enhance the immune function of patientsand effectively reduce the recurrent attack of eczema"Scutellaria-fresh Rehmannia-plantain-gentian-Dried tangerine peel-Poria cocos" can be used for acute and chronic eczema treatment.Therefore,the above 5 new prescriptions have different medicinal effects and covers almost all aspects of the eczema pathogenesis mechanism:hot,dampness,emptiness(breath,blood),dryness,stasis.The drugs of new prescriptions can be reduced and increased clinically in order to adapt patients' actual condition.(2)To analyze the prescriptions of Rehmannia glutinosa with the highest frequency.Through data analysis,we found that the most drugs compatible with fresh Rehmannia have the function of cooling blood,dehumidification and detoxification to stop itching drug.And with the highest frequency of combination with fresh Rehmannia were"Scutellaria-fresh Rehmannia "(44 times)," fresh Rehmannia-paeoniae "(43 times),"Fresh white skin-fresh Rehmannia "(41 times)," Sophora flavescens-fresh Rehmannia "(38 times).That the treatment of eczema,mainly by dampness heat and cooling blood,but more emphasis on qingrequshi.(3)We collected 100 external prescriptions in,involved 90 drugs.The top 14 drugs can be divided into 3 categories:heat-clearing and dryness(Phellodendron,Gypsum,Sophora flavescens,Fructus Kochiae,Scutellaria,Fresh white skin,Coptis chinensis),dry wet and insecticidal(alum,calomel,talc,fructus cnidii,pepper),heat-clearing and detoxification(Borneol,Indigo),therefore heat clearing is the main principle of external treatment of eczema.According to the association rules and cluster analysis of the drugs,we deduced 3-4 core drugs combinations;9 new prescriptions and 18 pairs.Combining the modern pharmacological research and the experience of clinicians,it was concluded that 5 prescriptions have certain clinical value."Oyster_fossil fragments_cuttlebone_Realgar" can be used for the convergence of eczema wounds and anti-inflammatory effects of swelling,according to the pharmacological analysis."fructus cnidii_Sophora flavescens-Fructus Xanthii_Fructus Kochiae_Fresh white skin " can be used to inhibit pathogens(such as Staphylococcus aureus),to combat eczema induced by delayed hypersensitivity." Phellodendron-betel-Coptis chinensis-talc-Indigo" has a similar function to the No.2 new prescription." Glycyrrhizae-Rhubarb-pearl powder-Pearl mother" has better results in chronic eczema accompanied by skin thickening.(4)we analyze and study the multi-component and multi-target complex reticular relationship of" Fresh white skin-Fructus Kochiae" drug combination.The results preliminarily concluded that the effective components of" Fresh white skin-Fructus Kochiae" drug combination play a role by RXRA,NCOA2,AR,RELA,TNF,IL-6these targets.Conclusion:For acute and subacute eczema,clearing heat and cooling blood are the main principles of treatment,Fresh white skin and fresh Rehmannia are common and important drugs for clinicians.According to the different pathogenesis mechanism of this disease,the prescriptions should plus and minus drugsand be flexible.In addition to dampness and heat,the treatmentfor chronic eczema should consider the weakness of a long time illness(blood deficiency,breathe deficiency),multiple stasis and dryness characteristics.Through the cluster analysis,9 new prescriptions were generated,and 5 core prescriptions were selected for clinical use.For the oral medicine,fresh Rehmannia is the most used drug,and the drugs compatible with the fresh Rehmannia is of cooling blood,dehumidification and detoxification to stop itching drug.This prescriptions weretformed on the basis of a change in the condition of the illness.The top 4 drugs combinations are "Scutellaria-fresh Rehmannia "," fresh Rehmannia-paeoniae "," Fresh white skin-fresh Rehmannia "," Fresh white skin-Coix seed".This showed that clearing heat and cooling blood is the main treatment principle for eczema.The main function of the external medicine in conjunction with the oral medicine,is to dry,kill insect and stop the itching,in order to improve the clinical symptoms.At the same time,external medication has therapeutic effect as well for chronic eczema.Therefore,according to clinical evaluations,the 4 core new prescriptions were selected for clinical use.Using the method of network pharmacology,we studies the complex reticular relationship of" Fresh white skin-Fructus Kochiae" drug combination to the treatment of eczema.The results of the study preliminarily verified the mechanism of "Fresh white skin-Fructus Kochiae" drug combination on eczema,and laid a good foundation for further exploring its mechanism.
