| Objective: the purpose of this study is to summarize the distribution of syndrome types by collecting the general data and TCM four diagnosis data of patients with down-regulation of long regimen of in vitro fertilization and embryo transfer(IVF-ET),and to analyze the regularity of Chinese medicine use after down-regulation by data mining,so as to provide a basis for TCM treatment intervention of patients with IVF-ET long regimen after down-regulation.Methods:(1)A total of 806 patients who underwent IVF-ET long-acting long regimen and short-acting long regimen from July 2019 to June 2020 at the Reproductive Medicine Center of Northwest Women’s and Children’s Hospital due to infertility were collected,and the patients’ basic information,life style and habits,past history,female physiology,reproductive history,infertility,menstruation,dysmenorrhea,general condition and the condition of tongue and pulse were investigated,Syndrome types are derived from the cluster distribution of syndromes.(2)Search the clinical literature published by CNKI,Wan Fang,VIP,CBM,Pub Med,and Medline from 1990 to the end of December 2020 on the treatment of traditional Chinese medicine intervention during IVF-ET long regimen down-regulation period,and screen the literature according to the inclusion and exclusion criteria.The amount and frequency of single Chinese medicine and its category,four qi,five flavors and channel tropism were analyzed by using Excel table,and the data model was established by using SPSS Modeler18.0 data mining software and Apriori algorithm of association rules.The association rules of the constituent medicine were analyzed,and the characteristics and laws of TCM at this stage were discussed.Results: 1.(1)The clustering results indicated that: 4 typical syndromes could be obtained,namely syndrome of deficiency of both yin and yang of kidney(325 cases,accounting for0.32%),the syndrome of liver depression and qi stagnation(215 cases,accounting for26.27%),the blood stasis syndrome(147 cases,accounting for 18.24%)and Phlegm-damp syndrome(89 cases,accounting for 11.04%).(2)In general,patients’ education level,years of infertility,and BMI index did not show differences in the distribution of TCM syndrome types;there were statistically significant differences in age,number of gynecological operations,pregnancy times,causes of infertility,BMI index and the distribution of TCM syndrome types(P<0.05).When the number of pregnancies≥2 times and the number of gynecological operations ≥ 3 times,the proportion of patients with kidney qi and yin deficiency syndrome after downregulation is significantly higher than that of other syndromes;among all causes,patients with IVF due to endometriosis were prone to blood stasis syndrome after down-regulation;patients undergoing IVF due to advanced age are likely to develop kidney qi and yin deficiency syndrome;and the ratio of deficiency of kidney qi and yin and phlegm-dampness syndrome in the patients with IVF due to ovulatory disorder is significantly higher than that of other syndrome types.2.(1)During the period of down-regulation and recuperation,a total of 18 kinds of Chinese medicines were used most frequently.According to frequency,they are Angelica,Cuscuta,Rehmannia glutinosa,Lycium barbarum,Dogwood,White peony,Chinese yam,Licorice,Atractylodes,Salvia,Poria,Cyperus rotundus,Ligustrum lucidum,Bupleurum,Codonopsis,Epimedium,Ziheche,Astragalus;(2)In the classified statistics of high-frequency drugs,tonic drugs were used 239 times,including qi tonic(70 times),blood tonic(81 times),yin tonic(38 times)and yang tonic(50 times),accounting for77.10% of high-frequency drugs;the drugs of mild propery in four qi were used 131 times,accounting for 42.26% of high-frequency drugs;the sweet drugs in the five flavors were used 244 times,with a frequency of 73.23%;the drugs belonging to the liver meridian were used for 227 times,with a frequency of 73.23%,including 190 times for nourishing the liver and 37 times for purging the liver;followed by the kidney meridian and spleen meridian,with a frequency of 55.16% and 57.42%,respectively.Based on the association analysis of high-frequency traditional Chinese medicine,a total of 53 association rules for drug pairs and drug groups were obtained,of which the support degree of dodder +Chinese wolfberry was the highest of 52.0%.Conclusion:(1)The long regimen of IVF-ET can be preliminarily divided into syndrome of deficiency of both yin and yang of kidney,Syndrome of liver depression and qi stagnation,blood stasis syndrome,and phlegm-dampness syndrome after down-regulation,in which kidney-qi and yin deficiency syndrome is the main syndrome;(2)The propery of medications for down-regulation are mainly sweet and mild,especially focusing on tonifying qi and blood,and replenishing the liver and kidney;(3)Dodder + medlar is the most commonly used drug pair during down-regulation,dodder-medlar-Rehmannia glutinosa-Angelica-Cornus-Yam is the most commonly used core drug group during the down-regulation period with the highest correlation. |
