Objective:Observation on the clinical effect of zusanli penetrate tiaokou acupoint to treat lumbar disc herniation(qi stagnation and blood stasis)Method:In this study,60 cases of lumbar disc herniation that meet the in clusion criteria were collected and divided into three groups according to random method.The basic treatment group,the conventional electro-acupuncture group,the elongated needle penetration group(hereinafter referred to as the basic group,electro-acupuncture group,awn needle group).Each group of 20 cases.The basal group was treated with so dium aescinate and thrombosis infusion therapy plus Chinese medicine package treatment.The electro-acupuncture group and the elongated needle group base on the basic treatment group。 For the electro-acupun cture group and the elongated needle group,took lumbar region jiaji point、shenshu point、 large intestine shu point、yaoyangguan、zhibian point、 ashi point in the back of waist;took huantiao point 、 fengshi Point、 chengshan point、 kunlun point、 zusanli point、 tiaokou acupoint in the affected limb.After acupuncture needle in electro-acupuncture group,took the huantiao point-the fengshi Point,chengshan point-ku nlun point、 zusanli point、 tiaokou acupoint.Two for a group of electro-acupuncture.For the elongated needle group,slowly needling at zusanli point,and along the stomach meridian of Foot Yangming down to tia okou acupoint penetration needling.Other acupoints took acupuncture needle.tookfour sets of electro-acupuncture after getting gas.Four groups of electro – acupuncture,one pole with a awn needle handle,and the other pole was connected to the huantiao、fengshi、chengshan and kunlun needle handle respectively.Both the two groups retained needle for 30 minutes after getting gas.Treatment once a day,Each treatment were 5 times.The interval between treatments is 1 day.Before treat ment,the first course of treatment and the end of the second course of treatment.The measurement of lumbar function and pain score for each patient were performed by the JOA Lower Lumbar Pain Rating Q uestionnaire and the Mace’s Pain Rating Table.Result:1 Comparison between the subjective symptoms,clinical signs and daily activity scores of JOA in three groups before treatment,and the difference was not statistically significant(P> 0.05).After the first and the second course of treatment,the three groups of patients with JOA subjective symptoms,clinical signs,daily activity limited score group were compared with before treatment.The basal group was sign ificantly lower than that of the electro-acupuncture group and the elo ngated needle group.The difference was statistically significant(P <0.01).After the first course of treatment,the JOA subjective symptoms,clinical signs and routine activity scores of the three patient groups in the basal group were significantly lower than those of the electro-a cupuncture group and the elongated needle group and the electro-acup uncture group was lower than those of the elongated needle group.The difference was statistically significant(P <0.01).After the second c ourse of treatment,the JOA subjective symptoms,clinical signs and routine activity of the three patient groups in the basal group were sig nificantly lower than those of the electroacupuncture group and the el ongated needle group,and the electro-acupuncture group was lower than those of the elongated needle group(P <0.05).Compared with the first course of treatment,the JOA subjective symptoms,clinical signs and routine activity scoresof the patients after the second course o f treatment were significantly increased.The difference was statistically significant(P <0.05).2 Before treatment,comparison of the Michael’s pain grading index,visual analogy classification and the existing pain intensity score in the three groups of patients.The difference was not statistically significant(P> 0.05).After the first and the second course of treatment,the three groups of patients with Michael’s pain grading index,visua l analogy classification,the existing pain intensity scores within the group were compared with the before treatment.The basal group was significantly higher than the electro-acupuncture group and the elongated needle group,The difference was statistically significant(P <0.01).After the first course of treatment,comparison of the three groups of patients with Michael’s pain classification index,visual analogy class ification,the existing pain intensity score.The basal group was signifi cantly higher than that of the electro-acupuncture group and the elong ated needle group,the electro-acupuncture group was lower than the elongated needle group.The difference was statistically significant(P <0.01).After the second course of treatment,comparison of patients with Michael’s pain grading index,visual analogy classification method,the existing pain intensity score.The basal group was significantly higher than that of the electro-acupuncture group and the elongated needle group,the electro-acupuncture group was lower than the elongated needle group,the difference was statistically significant(P <0.05).Co mpared with the first course of treatment,the pain rating index,visual analogy method and the existing pain intensity score of the patients after the second course of treatment were significantly lower(P <0.05).3 Comparison of the overall clinical efficacy(markedly effective rate)between the three groups,the difference was statistically signifi cant(P <0.05).The acupuncture group and the acupuncture group were superior to the basal group.Electro-acupuncture group and elongated needle group were superior to the basic group,and the elongated ne dle group was superior to the electro-acupuncture group(P <0.05).In dicated that the efficacy of elongated needle zusanli penetrate tiaokou acupoint was the best.4 Three groups of patients in the course of treatment did not appear acupuncture accident and adverse reactions.There were also no adverse reactions during follow-up.Conclusion:Three methods of the basic treatment,conventional electro-acupun cture treatment and elongated needle treatment were all effective in the improvement of lumbar discherniation(qi stagnation and blood stas is)in patients with JOA points,pain scores and the overall clinical eff icacy,and no adverse reactions.Theeffect of the elongated needle pen etration therapy was better than the conventionalelectro-acupuncture therapy and basic therapy.
