Objective:Investigate that effect of home noninvasive positive pressure ventilation?NIPPV?on pulmonary rehabilitation in patients with chronic obstructive pulmonary disease?COPD?.Methods:From March 2015-December 2017 in the first affiliated hospital of Xinjiang medical university center for respiratory and critical care medicine diagnosis for plateau 41 patients with severe COPD,diagnostic criteria in plateau GOLD III-IV criteria for the diagnosis of COPD.The inclusion standard PaCO2?6.7kpa?50.0mm Hg?or pH<7.35.Randomly divided into intervention group?21 cases?and control group?20 cases?and control group given conventional oxygen cure+routine drug treatment,the intervention group was given conventional oxygen cure+conventional drugs+NIPPV treatment,NIPPV based PaCO2 regulating target to reduce at least 10%or PaCO2<6.5 kPa?48.75 mm Hg?.Two groups of patients were enrolled for at least 12months.Collect 2 set of demographic characteristics,blood gas and lung function,6-MWD,SGRQ,CAT,and statistical analysis.Results:There were 17 cases in the intervention group and 18 cases in the control group.There was no statistically significant difference between the two groups in baseline data?P?0.05?.Intervention group 6months,12 months of lung function index,CAT and 6-MWD,SGRQ,ompared with the baseline,there were no statistical differences?P?0.05?,6or12months 6-MWD,SGRQ and its basic value is statistically significant?P<0.05?.The comparison of two groups of index change?6 months and 12 months respectively with the comparison between the difference of the baseline value?,in addition to the delta CAT 6 months and 12 months and12 months delta SGRQ statistically significant?P<0.05?,the rest were no statistical difference?P?0.05?.Conclusion:NIPPV,which aims to reduce PaCO2,cannot improve lung function of patients with stable severe COPD,but can improve patients'symptoms,improve patients'quality of life and facilitate patients'pulmonary rehabilitation.
