Objective: To investigate the therapy effects of HFO(high flow oxygen)and NPPV(Noninvasive plsitive pressure ventilation)in acute exacerbation of chronic obstructive pulmonary disease(AECOPD)in elderly patients.To evaluate the advantage of HFO as a new way of treatment of AECOPD.Methods: One hundred patients diagnosed with AECOPD during ICU2015.01-2016.01 were randomly divided into HFO and NPPV two groups,50 cases were included in each group.All patients were given HFO and NPPV,atomization inhalation after anti-infection and relieving asthma therapy.The indexes of blood gas analysis,the clinical symptoms improved situation,aeration time,oxygen cure time,hospitalization days,mortality and the incidence of various complications were observed.Curative effects of HFO and NPPV in elderly patients with AECOPD.All data were treated by SPSS18.0.The inspection data first to investigate whether belong to normal distribution.Metrics to mean +/-standard deviation,using the single factor analysis of variance,if you have rate data by chi-square analysis.Results:1 50 cases of HFO group,50 cases of NPPV groups,two groups of hospitalized time,heart rate,breathing rate,blood routine,blood pressure,and pulmonary function(FEV1/FVC,FEV1%)and distribution difference had no statistical significance(P>0.05).2 PaO2,PaCO2 of two groups of blood gas analys has no statistical significance(P>0.05)at the beginning of treatment(0h).PaO2 were improved significantly in both groups 12 h and 5d after treatment(P<0.05),but each time point in the two groups had statistically significant(P<0.05),but the time difference between the two groups had no statistical significance(P>0.05).3 No obvious difference were found between two groups of patients about ventilation time(P>0.05),hospitalization time and significant statistical difference(P>0.05).4 There were no significant differences in propagatde,HGB,NEY%,the WBC between two groups(P>0.05),but in two groups(P<0.05).5 No difference were found in the incidence of complications,in two groups(P>0.05).6 The patients' comfort,cost during hospitalization,medical therapy time had statistically significant(P<0.05)7 FEV1 / FVC and FEV1 % in two groups before and after treatment had statistical significance(P<0.05),no obvious difference between the two groups(P>0.05)Conclusion:1 Blood gas analysis results were improved significantly before the treatment in HFO and NPPV group,with two noninvasive ventilation modes in the treatment of AECOPD had good treatment effects,can improve significantly oxygenation and ventilation function.2 Application of HFO compared with NPPV,the incidence of complications,hospitalization days,computer time had no differences between the two groups(P>0.05).3 Compared with NPPV,there were significant improvement on medical therapy time in comfort and cost of HFO.4 HFO has little injury,tolerance,is one of the preferred model for the treatment of AECOPD.
