Objective:To explore the effect of mechanical ventilation timing on the survival rate,length of hospital stay and ventilation time of patients with AECOPD and ARF.Methods:1.The research object was from January 2017 to August 2019.The first diagnosis of AECOPD was 1797 patients in the Department of Respiratory and Critical Medicine of Nanhua Hospital Affiliated to Nanhua University,including469 patients with ARF.Collect the clinical data of the research subjects,including the general condition of the patients,laboratory data and hospitalization days,ventilation time,mortality,and failure rate?the ratio of noninvasive to invasive?.2.The subjects were divided into the first group:54 cases of noninvasive ventilation when PaO2?70mmHg;221 cases of noninvasive ventilation when PaO2?60mmHg.The second group:there were 19 cases of invasive ventilation when PaO2?50mmHg;43 cases of invasive ventilation when PaO2?40mmHg.3.Data analysis uses SPSSForResults:1.In AECOPD combined with ARF,the number of hospital stays and non-invasive ventilation with PaO2?70mmHg during sequential treatment were less than those with PaO2?60mmHg for noninvasive ventilation.The difference was statistically significant?P<0.05?.2.Among AECOPD and ARF,the number of hospital stays and ventilation time when PaO2?70mmHg was selected was less than that of PaO2?60mmHg patients.The difference was statistically significant?P<0.05?.3.In AECOPD and ARF who underwent sequential treatment,the number of hospital stays and ventilation time when PaO2?50mmHg were selected were shorter than those when PaO2?40mmH were performed.The difference was statistically significant?P<0.05?.4.The comparison of WBC,CRP,BNP,and SBP at each time point before and after ventilation in patients with NIPPV and IPPV were statistically different?P<0.05?.The BNP in the IPPV group decreased more significantly than in the NIPPV group.5.The N%,PCT,and DBP in the NIPP group at various time points after ventilation were lower than those before ventilation.The difference was statistically significant?P<0.05?.There was no statistical difference in these indicators in the IPPV group?P>0.05?.6.Patients in the IPPV group and the NIPPV group had improved pH,PaCO2,and PaO2 after ventilation,and the differences were statistically significant?P<0.05?.These indicators improved most significantly at 6 hours after ventilation.Conclusions:1.In patients with AECOPD and ARF,compared with PaO2?60mmHg,noninvasive ventilation was performed when PaO2?70mmHg,which significantly improves the short-term survival rate,shortens the length of hospitalization and ventilation time.2.In patients with AECOPD and ARF,compared with PaO2?40mmHg,PaO2?50mmHg for invasive ventilation significantly improved the short-term survival rate,shortened the length of hospitalization and ventilation time.
