The Study On Knowledge,Attitudes,Practices And Factors Of Community Doctors Toward Pneumococcal Vaccination In Hangzhou

Objectives1.To evaluate the Knowledge,Attitudes,Practices of pneumonia and pneumococcal vaccine in community doctors in Hangzhou.2.To evaluate the community doctors' willingness to recommend pneumococcal vaccination for the elderly in Hangzhou,and analyze the factors that influenced the doctors' willingness.MethodsSystem cluster sampling was used and twenty community health service centers were selected from the five main urban districts of Hangzhou.Doctors who engage in medical activities were selected as this study's subjects.Subjects were asked to fill in a pre-designed questionnaire,"Questionnaire on community doctors' knowledge of senile pneumonia and recommend willingness of pneumococcal vaccination for the elderly."To evaluate the knowledge,attitudes and practices of community doctors to pneumococcal vaccine,and analyze the influence factors in Logistic regression analysis.The questionnaires were double-input and established by Epidata3.2,and SPSS22.0 was used for statistical analysis.P<0.05 was considered statistically significant.Results1.A total of 870 subjects were selected in this study.The number of general practitioner was 549 and 321 for specialist doctor.2.The community doctors' approve rate of "elderly was vulnerable to pneumonia"and "senile pneumonia was serious" was 97.24%and 97.47%respectively,the agreement rate of pneumococcal vaccine's effectiveness and safety was 95.40%and 95.52%respectively.68.97%of community doctors had suggested elderly to inoculate pneumococcal vaccine in their daily work,and 62.41%of them were willing to suggest elderly to inoculate pneumococcal vaccine in the future.3.The department of doctors(OR=0.365,95%CI:0.205?0.648,P=0.001)and the severity of senile pneumonia they considered(OR=5.213,95%CI:1.683?16.150,P=0.004)was the influence factors of community doctors' knowledge toward pneumococcal vaccine.Vaccine-related events(P<0.05)and the knowledge of senile pneumonia(P<0.05)was the influence factors of community doctors' attitude toward pneumococcal vaccine.The gender(OR=1.566,95%CI:1.114?2.202,P=0.010),department(OR=0.434,95%CI:0.312?0.604,P<0.001),working years(P<0.05),vaccine-related events(OR=0.280,95%CI:0.053?0.722,P=0.035)and health care insurance rules(OR=4.294,95%CI:3.033?6.079,P<0.001)was the influence factors of community doctors' practice toward pneumococcal vaccine.4.The department of doctors(OR=0.383,95%CI:0.166?0.887),the knowledge(OR=2.983,95%CI:1.132?8.972),attitude(OR=4.321,95%CI:1.365?13.680)and practice(OR=10.827,95%CI:5.671?20.670)toward pneumococcal vaccine,and health care insurance rules(OR=2.051,95%CI:1.087?3.871)was the influence factors of the community doctors' willingness recommend pneumococcal vaccination for the elderly.Conclusion1.The present situation of community doctors to recommended and recommended willingness for elders of pneumococcal vaccination is poor in Hangzhou.2.Training of senile pneumonia and pneumococcal vaccine-related knowledge for community doctors should strengthen,and doctors in different departments need to carry out targeted training.3.The health care insurance rule of account balance to pay the cost of pneumococcal vaccination need widely publicize.The free pneumococcal vaccination for elders is recommended.4.Community doctors should take medical knowledge to treat vaccine-related events correctly and objectively.
