Investigation And Analysis On Knowledge, Attitude, Practice Toward Cesarean Delivery Of Obstetrician

Objective:To assess the knowledge,attitude,practice toward cesarean delivery of obstetrician,assess the influence to the rate of cesarean delivery,and analyze the influencing factors of making the cesarean delivery decision for the doctors.Methods:62 obstetricians who come from the Shanxi Maternal and Child Health Hospital,Taiyuan Maternal and Child Health Hospital and Linfen Xianping Maternal and Child Health Hospital had been investigated by the self-designed questionnaire of "the knowledge, attitude,practice toward cesarean delivery of doctors questionnaire".The rate of cesarean delivery had been calculated.We analyzed the associatively between them by SPSS13.0 software,assessed the influence to the rate of cesarean delivery,and analyzed the influencing factors of making the cesarean delivery decision for the doctors.Results:1.Tendency of the rate of cesarean deliveryFrom 2002 to 2006 the rate of cesarean delivery were 39.1%,40.8%,43.6%,44.4%,45.8% respectively.It was increased year by year in the passed five years.2.The knowledge of cesarean deliveryThe knowledge of cesarean delivery of obstetricians were abundant overall,the percent of the doctors who have objective cognition on the indication of cesarean delivery is 91.9%.The percent of the doctors who know the current situation of cesarean delivery is 80%.And almost doctors know advantages and disadvantages of cesarean delivery.3.The attitude of cesarean deliveryMost of the obstetricians would like to choose spontaneous delivery in the case of the safety of maternal and infant.The level is 95.1%.But doctors were worried about the medical dispute caused by disagreement with the cesarean delivery requested by gravida.The proportion of the all doctor is 38.8%.4.The practice of cesarean deliveryMost of the doctors would choose the deliver way according to the indication.However,in order to avoid the medical dispute,most of them prefer to choose the cesarean delivery.The percent of them are 87.1%and 87%.5.The relationship between the doctor's age,culture,the term of job,the degree of professional and the cesarean delivery's knowledge,attitude,practiceAbout the cesarean delivery's knowledge,attitude,practice,there are no significant deviation on age of obstetrician.F value are 0.835,0.753,0.895,P＞0.05:About the cesarean delivery's knowledge,practice,there are significant differents on culture of obstetrician,F value are 3.768,4.598,P＜0.05:About the cesarean delivery's attitude,,there are no significant differents on culture of obstetrician,F value are 2.551,P＞0.05.The longer of the term of job,the higher of score on the cesarean delivery's knowledge,attitude,practice,The F value are 6.236,6.009,3.426,P＜0.05.the higher degree of doctor the,the higher of score on the cesarean delivery's knowledge,attitude,practice,The F value are 3.367,4.257,4.815, P＜0.05.6.Correlation analysis of the rate of cesarean delivery,knowledge,attitude,practiceThe knowledge of cesarean delivery,the attitude and the practice were positive correlated with each other(P＜0.01).The rate of cesarean delivery were correlated with the knowledge, attitude,practice(P＜0.01).The coefficient correlation are 0.518,0.398,0.323.7.Principal component analysis of the influencing factors of making the cesarean delivery decision for the doctors.The doctors were mainly influenced by consequence factors,pregnant women factors and colleague factors when making the cesarean delivery decision.Conclusions:The rate of cesarean delivery were increased year by year in the passed five years.There are positive correlation between the cesarean delivery knowledge,attitude and practice of the obstetricians.The rate of cesarean delivery were correlated with the knowledge,attitude and practice of doctors.The doctors were mainly influenced by consequence factors,pregnant women factors and colleague factors when making the cesarean delivery decision.
