| Objective:According to the current research of electronic bowel preparation quality before colonoscopy diagnostic and treatment,combined with clinical practice,tried to develop a scale called “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”,and evaluated reliability and validity of the scale,then applied the scale to the in patients of colonoscopy.Preliminary established “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”.Methods: 1.Study site and objectsSite were chosen in a Third Grade & Class Hospital in Yancheng.Research objects were selected by criterias: 1)age 18 ≥ years;2)no cognition and language communication barriers.2.Scale organization and appraisal programsAccording to rules of organizing scales,used literature review,experts survey,Delphi Expert Consultation and small sample pre-test to form items of “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”,and used method of variative coefficience to assign values for every item,and used methods of Krone Bach Coefficient,Split-Half Reliability to test the reliability of the scale;used methods of content validity and structure validity to test the validity of the scale.3.Clinical Application Practice of “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”Measured the consistency between scores of “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment” and scores of Boston Scale and Ottawa Scale,which is using frequently in evaluating bowel clearance in Electronic Colonoscope.Chose 220 patients which were in Department of Gastroenterology from Sep.2016 to Oct.2016 randomly,and scored them with new scale,Boston Scale,and Ottawa Scale.Measured the possible psychological effect of “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”.Chose 69 patients which were in Department of Gastroenterology randomly,then put them into experimental group and control group randomly.Samples in experimental group were scored by new scale and Boston Scale,and samples in control group were only scored by Boston Scale.Compared the scores of Boston Scale of samples in different group,in order to know if there is any difference between them.Result 1.Characters of scale22 items were organized to compose the “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”.2.Investigation of scale2.1Feasibility of scale20 industry experts were invited in the research;20 questionnaires were sent out and returned,with the return rate of 100 percents.In clinic,All the questionnaires were completed in 3 to 5 minutes,and the average in 4 minutes;the time of scale completion were within the acceptable range of patients.2.2 Reliability of scaleCronbach α coefficient,Split-Half Reliability were used to test the scale reliability.In the research,the results shows: Cronbach α coefficient of whole scale was 0.910,and the Cronbach α coefficients of subscales were between 0.791 to 0.819.The reliability results showed that the items in the scale had a high degree of internal consistency.2.3 Validity of scaleContent Validity: During the course of editing scale,two rounds of Delphi Expert Consultation were carried out,and items were filtered,added or deleted;through statistical calculations,such as means and coefficient of variation,weight coefficient of each item in scale were confirmed,and each item can basically guaranteed to express the content as it were designed.Structure validity: after the data of scale were analyzed by factors,extracted 6 common factors according to the cumulative contribution rate above 70 percent,and latent root≥1.The result shows: cumulative contribution rate were 71.516 percent,and factor loads of each item were greater than 0.6 and no item has the same load conference between two factors.The scale showed good validity.3.Clinic application research of scaleThe granding result of “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”is basically consistent with Boston Scale and Ottawa Scale.The full mark of “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”is 100 points,and was divided into four grades:score in 76-100 was regarded as excellent;score in 51-75 points was regarded as good;score in 26-50 points was regarded as ordinary;score in 0-25 points was regarded as poor,those who score above 50 are qualified.220 cases of patients with “Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”score and Boston scale and Ottawa scale score were completely consistent with the results of 203 cases,consistency is higher than 92%.Conclusion1.This study follows the principles and methods of establishing scale,Study the relevant literature,expert investigation,clinical pre-test,To ensure the development of the scale and the quality of the scale.2.“Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment”includes 22 items,Basically covers all the contents of bowel preparation before the electronic colonoscopy,To ensure the integrity of the scale.3.In process of establish the scale had two rounds of Delphy expert advice,and through the mean,coefficient of variation to determine the weight of the items,and used methods of Krone Bach Coefficient,Split-Half Reliability to test the reliability of the scale;used methods of content validity and structure validity to test the validity of the scale,It shows that the scale is scientific.4.“Assessment Scale of Bowel Preparation for Nursing in Electronic Colonoscope Diagnosis and Treatment ” a preliminary assessment of intestinal cleanliness before colonoscopy,improve the intestinal cleanliness in colonoscopy,avoided or reduced the pain caused by secondary colonoscopy. |
