ObjectiveThis research is about systematic retrospective analysis of upper-gastrointestinal diseases done by gastroscopy from 2005.01 to 2007.12 in Zhongshan hospital,performing stepwise study of their incident rate,age distribution,gender ratios as well as prevention and treatment strategies of upper-gastrointestinal diseases in recent 3 years.It provides reliable clinical data to make health policy for upper gastrointestinal diseases prevention. At the same time,it also helps clinicians understand upper gastrointestinal diseases,its clinical epidemiological characteristics,and for further development of upper gastrointestine endoscopic examinations to provide references.Following socio-economic development,changes has been appeared in upper gastrointestinal diseases.Moreover,in recent years,there are no large-scale researches in the endoscopic examination of these diseases,so it was necessary to perform retrospective analysis of upper-gastrointestinal diseases of recent 3 years examined by gastroscopy in Zhongshan hospital and acquired preliminary understanding of changes in GI diseases detected in gastroscopy.MethodsThere are altogether 44480 patients of upper gastrointestinal diseases evaluated by endoscopic tests in Fudan University,Zhongshan Hospital from 01.2005 to 12.2007 determined as per Period of Time,Sex,four Age groups in years of old(0~20,21~40,41~60,≥61).OLYMPUS-XQ240-1 model or OLYMPUS-XQ260-1 model instrument was used for electronic endoscopic tests.For all of these cases of different main upper gastrointestinal diseases(Chronic gastritis, Duodenitis,Upper gastric Ulcer,Upper GI polyp,Esophageal varices,Esophageal cancers, Stomach cancer,Duodenal cancer,and other diseases),performed group description, comparing differences in between groups.Among them upper GI Ulcers include:Gastric ulcer,Esophageal ulcer,Duodenal ulcer,Gastro-duodenal multiple ulcer,and Anastomotic ulcer.Upper GI Polyp includes:Esophageal polyp,Cardiac polyp,Gastric polyp, Duodenal Polyp.Others comparatively less common diseases are:Billiary reflux, Esophageal hiatal Hernia,Residual gastritis,and Anastomotitis.Each groups of diseases as per basic characteristics like sex,age are proceeded to descriptive analysis,besides comparing the differences of male and female group by four age fragments.ResultsTotal of 44,480 cases,excluding 114 cases,among the actual statistics for the 44,366 cases,male:22,658,female:21,708,M:F=1.04:1.The youngest is 10 years old,the oldest is 88 years old.Among them,chronic gastritis 24,490 cases(55.20%,male:9,771,female: 14,719,M:F=0.66:1,the main incident age is 41~60 years old.Esophagitis 3,035 cases (6.84%),male:1,888,female:1,147,M:F=1.67:1,the main incident age is 41~60 years old.Duodenitis 2,221 cases(5.01%),male:1,584,female:637,M:F=2.48:1.the main incident age is 41~60 years old.Upper GI polyps 2,205 cases(4.97%),male:869, female:1,336,M:F=0.65:1,the main incident age is 41~60 years old.Upper gastric ulcer 6985 cases(19.50%),male:4743,female:2242,M:F=2.12:1.the main incident age is 41~60 years old.The detection rate of gastric ulcer which account for 4.96%, M:F=2.10:1.Esophageal ulcer detection rate of 0.24%,M:F=1.35:1.Gastroduodenal ulcer compound rate accounted for 0.91%,M:F=3.38:1.Duodenal ulcer detection rate of 7.75%,M:F=1.89:1.Duodenal ulcer-like creamy spot detection rate of 1.62%,M:F= 2.89:1.Esophageal varices 1387 cases(3.13%),male:449,female:438,M:F=2.17:1. Esophageal cancer 616 cases(1.39%),male:469,female:147,M:F=3.19:1,age on the incidence is of≥61-year-old patients.Gastric cancer 1485 cases(3.35%),male:1067, female:418,M:F=2.55:1,age on the incidence is of>61-year-old patients.Duodenal cancer 16 cases,male:14,female:2,as these cases are very few,being statistically insignificant.There are other diseases(bile reflux,residual gastritis and anastomotic inflammation,residual gastric cancer,hiatus hernia) of total 1,926 cases,male:1,304, female 622;M:F=2.10:1.ConclusionsAmong upper GI diseases,benign diseases are mainly detected.Among benign diseases,detection rate of chronic gastritis is the highest,the detection rate of chronic gastritis is highest in female than that of male(P>0.05);The next is upper GI ulcer.The peak incident age for upper G1 ulcers is in between 41 to 60-year-old,no matter what type of upper GI ulcers are,males are obviously more than females(P<0.028).The incident rate of duodenal ulcer was significantly higher than that of gastric ulcer,accounting for 7.75%.Comparing with domestic report,the ratio is 1635/19806(8.25%),shown a downward trend.Worthy of attention is on the incidence of upper gastrointestinal polyps are 4.97%.Esophageal varices incident rate was 3.13%,more male than female,we also found that male and female incidence peak age is different,male found at the peak age of 41 to 60 years old,the peak detected in female aged≥61 years old;Among malignant diseases,the detection rate of gastric cancer is the highest,followed by esophageal cancer. Among sex group,the detection rate of esophageal and gastric cancer is highest in Male than that of Female.The peak incident age for esophageal and gastric cancer is>61 years of old.
