Study On Quality Of Life And Influencing Factors With Neck And Back Pain Among Xiji County Of Ningxia

Objective: To investigate the status of the pain and other relevant circumstances in patients with neck and back among 5 townships in Xiji County of Ningxia,and to explore the rural neck and back pain patients life quality status and its influencing factors.It is suggested that the management and early intervention of the rural residents' pain should be taken to help to delay the development of the disease.In order to improve the rural neck and back pain patients' quality of life,putting forward the management of the rural residents' pain and early intervention.This study aimed to provide the scientific basis to develop rural health work,rural health policy and measures,and further health service system.Methods: Two-stage sampling method was used in this study.The first stage adopted typical sampling survey method to select 5 townships from 19 townships in Xiji County(Xin Ying Township and Bai Cheng Township and Hong Yao Township and Ma Jian Township and Tian ping Township).The second stage by using cluster sampling method to investigate 5 townships include 47 village of the neck and back pain patients with face-to-face survey.Linear correlation was be used to analyze the realationship between intensity of pain and the quality of life.Multiple stepwise regression analysis was be used to explore the influencing factors of the quality of life.Results:(1)The survey participants included 1180 patients with neck and back pain,The average age was 56.90±12.02 years old,the median age was 57 years old.There were 709 women(accounting for 60.1%),471 men(accounting for 39.9%).(2)The total score of the quality of life in rural areas for patients with neck and back pain among Xiji County was 55.82±10.69,physical health score was 48.54±12.25,mental health score was 59.98±16.21.The score of eight dimensions of quality of life in patients with neck and back pain were significantly lower than the Chinese norm(p<0.05).Compared with the norm of age,eight dimension scores in addition to the 65 and older age groups had no statistically significant difference on mental health,dimension scores of vitality was higher than the age of norm,the rest of the dimension scores were lower than the norm(p < 0.05);Compared with the norm of different gender age,the quality of life of patients with neck and back pain scores in eight dimensions were significantly lower than the norm of gender(p<0.05).(3)The pain of patients with neck and back pain in Xiji was showed severe stabbing pain,cramps referred pain and tenderness.Patients treated their neck and back pain in primary medical institutions for the county hospital,and they always chose outpatient treatment,their choice mainly affected by the distance,technology and cost.The majority of patients had a tolerance for pain,but they were more likely to choose outpatient treatment for pain relief.(4)Multiple linear regression analysis showed that gender,age,PPI,PRI score,income,duration of disease,and family population were the main factors affecting physical health(P<0.05).Gender,age,PPI,PRI score,PRI emotional items,education level,family population and the stability of the disease had significant effects on mental health(P<0.05).Gender,age,PPI,PRI score and course of disease were the influencing factors of the quality of life(P<0.05).Conclusion: The quality of life in patients with neck and back pain among Xiji was poor,their mental health was better than physical health,patients often used drugs to treat diseases,but the effect was often transient,and drugs cannot effectively controlt hedisease.Good economic condition was an important guarantee for the quality of life,improving the living standards of rural residents can perfect their quality of life,pain was negatively correlated with quality of life,the greater the pain was,the worse the quality of life be.So grass roots should be focused on pain,and give full play to the advantages of the grassroots medicine,effective control and management of the pain c an help neck and back pain symptoms to relieve the influence of physiological,psych ological aspects and then improve the quality of life.
