Objective:The study aimed to explore the prevalence of chronic low back pain(CLBP)and back pain in patients with functional dyspepsia(FD),and investigate the effects of CLBP or back pain on dyspepsia symptoms and quality of life in FD patients.Methods:It was a cross-sectional and longitudinal follow-up study,this study focused on all patients with FD,with or without CLBP or back pain,but without psychiatric history.Somatization was measured by Patients Health Questionnaire-15(PHQ-15),Adapted Patients Health Questionnaire-15.Nepean Dyspepsia Index(NDI)was used to assess life quality.Dyspepsia Symptoms Severity(DSS)was also applied.Depressive was carried out by Patient Health Questionnaire-9 scale(PHQ-9),and Generalized Anxiety disorders-7 scale(GAD-7)was filled out to detect anxiety disorders.After 1year follow-up,the patients reported the change of their condition:disappearance,improvement,no change,worsening.chi-square test,analysis of variance,spearman correlation analysis,binary logistic regression and linear regression analysis were used to analyze the influence of CLBP and back pain on FD patients?dyspepsia symptoms and quality of life.Results:The prevalence of CLBP and back pain was 7.1%and 26.9%respectively.There were statistically significant differences between the two groups(?~2=39.368,P=0.000).FD patients accompanied with back pain and chronic low back pain were followed up,and the condition disappeared,improved,no significant changes and worsened were observed,with13(13.5%),61(63.5%),10(10.5%),12(12.5%)cases respectively,non-pain group were 43(23.1%),128(68.8%),8(4.3%),7(3.8%)cases respectively,and there was a significant differences between the three groups on the change of condition(?~2=13.838,P=0.003).The age group(?~2=19.182,P=0.001),anxiety(F=5.511,P=0.001),depression(F=5.291,P=0.001),adapted PHQ-15(F=8.741,P=0.000),PHQ-15(F=12.136,P=0.000),NDI(F=4.900,P=0.002)were significantly different in the non-pain group,back pain and CLBP group of FD patients,and anxiety(r=0.185,P=0.002),depression(r=0.224,P=0.000),adapted PHQ-15(r=0.227,P=0.000),PHQ-15(r=0.277,P=0.000),NDI(r=0.178,P=0.003),classification of FD(r=0.150,P=0.011)were associated with back pain.A binary logistic regression analysis was performed on the related factors of back pain.the results showed that somatization(OR=1.173,P=0.000),NDI(OR=1.064,P=0.000)were factors affecting back pain,the overlapping group with a higher incidence of back pain,and depression has a tendency to affect back pain(OR=1.074,P=0.066);Linear regression analysis was performed on the related factors of NDI before follow-up,the results suggested that back pain(?=0.428,P=0.001),anxiety(?=0.113,P=0.006),depression(?=0.096,P=0.04),somatization(?=0.369,P=0.001)are the influencing factors of it.Conclusion:Back pain is more common than chronic low back pain in FD patients,Back pain is associated with somatization,anxiety and depression,and it is related to quality of life in FD patients.Follow-up compared with control group after treatment,the improvement of symptoms was more obvious in FD patients without CLBP and back pain.
