| Objective: To analyze the trend of disease spectrum of hospitalized patients with liver diseases from 2002 to 2015 in the Yijishan Hospital of Wangnan Medical College a-nd explore the cause of the change of disease spectrum,and to provide an evidence-based basis for liver diseases prevention,treatment and management.Methods: This research was a retrospective study.We collected 8301 patients with liver disease from Yijishan Hospital between January 2002 and Decemble 2015,whose first diagnosis were viral hepatitis,drug-induced liver disease,alcoholic liver disease,autoimmune liver disease,fatty liver,schistosomiasis liver disease.The spectrum changes of liver diseases and age,sex distribution were analyzed prospectively by SPSS 19.0 software.Results: In 2002-2015,viral hepatitis is the majority of the spectrum of Liver disease,followed by drug-induced liver disease,alcoholic liver disease,autoimmune liver disease,fatty liver,schistosomiasis liver disease and other liver diseases.Their respectively ratio in the spectrum is 77.16%、5.88%、3.02%、2.31%、1.30%、1.28%、9.05%.During this years,the total number of hospitalized patients with liver disease in the hospital is increasing year by year.The number of hospitalized patients with viral hepatitis has been ranked first in other liver diseases in each year,but the proportion of hospitalized patients with liver disease showed a continuous downward trend.The ratio is more than 70%.In different years,the number of hospitalized patients with drug-induced liver disease increased year by year,but its ratio in the patients with liver disease was relatively stable,and remained at about 5%;The proportion of alcoholic liver disease showed a rising tend after the first decline,and up to 5.65%.In the patients with viral related liver diseases,HBV associated liver diseases occupied the majority constituent ratio in all patients with viral related liver diseases,which accounted for 86.63%,but the ratio of patients with viral hepatitis hospitalized showed a decline tend in general;The cuver of viral hepatitis C showed an upward trend in 2002-2011 and a slightly drop after 2011;Viral hepatitis A and viral hepatitis E showed a decline trend.In patients with viral hepatitis B,chronic hepatitis B accounted for 45.02%,but its ratio showed a decline trend.The proportion of patients with acute hepatitis B in patients with hepatitis B virus showed obvious decline trend,which proportion decreased from 20% to 5.42%;The liver cirrhosis and primary liver cancer related HBV showed an upward trend.In this hospital,male liver disease suffers were more than female in addition to autoimmune liver disease and drug-induced liver disease.Alcoholic liver disease Male 97.61% and male to female ratio was 40.8:1;Autoimmune liver disease Female 86.98% and male to female ratio was 1:6.68.Liver diseases were concentrated in the middle-aged(41-65 years).Viral hepatitis was concentrated in the young adults(age 18 to 40),middle-aged(41-65 years),they individually accounted for 33.47%,52.05%;Alcoholic liver disease mainly concentrated in the middle age(73.31%),followed by the elderly(13.94%)and the young adults(12.75%).Conclusion: Liver disease spectrum has some changes in this hospital from 2002-2015.Viral hepatitis,although in the first place,but its proportion to the overall has showed decline trend.It is noteworthy that the proportion of HCV is rising.In patients with viral hepatitis B,the proportion of acute viral hepatitis B is declining.Autoimmune liver disease is more common in middle-aged women,and other diseases are more common in middle-aged men. |
