Study On The Situation And Classification Of Community Public Health Service Projects In Urumqi

Objective:To explore the problems in community health work by studying the basic situation of four community health service institutions in Urumqi and the development of public health service projects.Combining the human,financial and public health service of community health institutions,To carry out the classification of basic public health services and clarify the focus of community public health work,standardize the basic functions of the community.Methods: The basic situation of the four communities and the status of public health services to conduct a descriptive analysis,with the literature,Delphi method,ABC classification of public health services to classification.Results:(1)2008~2015,Urumqi City,four community health service center public health expenditure accounted for the proportion of total annual revenue increased year by year.Health technical staff to specialist-based,title to primary and intermediate-based,per million people have a general practitioner and public health physicians are less than 1 person.(2)National basic public health service projects in Urumqi community are carried out in a comprehensive manner,the implementation of the project slightly different results.The number of health education materials increased by the community;newborn visits and management rate is higher,more than 60%;0 to 3 years old children per capita The management rate and the management rate of outpatients were higher than 50%;the registration rate of patients with type 2 diabetes mellitus was higher than that of the control group The overall rate of increase,the management rate and standardized management rates are more than 60%;severe mental illness patients registration rate,the management rate is generally more than 75%;community health service centers to assist in the number of health supervision inspections generally small,Once a year;tuberculosis patient health management,the elderly constitution identification and children's traditional Chinese medicine nursery service few residents use.(3)According to the economiccharacteristics of community public health services classification,the community public health services are divided into 16 public goods and quasi-public goods 30.According to the results of two rounds of Delphi expert consultation,the positive coefficient of expert opinion was 100%,the authority coefficients were 0.78,0.80(all greater than 0.7),the coordination coefficient was 0.31~0.49(P <0.01).According to experts on the importance of public health services classification of views,the service is divided into very important,more important,generally important,less important and no important.From the experts on the community public health service capacity and quality of service views,provided and guaranteed the quality of the project,the ability to upgrade to ensure the quality of the project,can't guarantee the quality of service projects.Classification of services by ABC classification,A are the most important service content and the community has the ability to provide,and the completion of high quality of service projects,the highest priority implementation;B are the more important services,but also the community can provide,but through training and other measures to improve service capacity in order to achieve the quality of service requirements to meet the requirements of the project,the priority implementation;C are the experts that the general importance,but the community in the current conditions Under the constraints of legal,technical and other factors can not provide or can not guarantee the quality of service projects,the final implementation.Conclusion:(1)Urumqi community health institutions run better,but the health technical staff of the academic qualifications and titles are generally low,the shortage of general practitioners and public health physicians,health force team structure needs to be adjusted.(2)Community public health services carried out in good condition,but faced with insufficient financial investment,poor human resources structure,public health service efficiency to be improved and so on.(3)According to the classification results,public goods are provided free of charge to the residents by government finances;quasi-public goods can be partially charged by the residents who use the services.A class of the highest priority implementation of public health services projects should be fully covered,to strengthen the inspection and assessment,give priority to increase their capital investment,so that community health personnel to quality and quantity to complete these services,is the focus of public health work;Service should strengthen and improve the community infrastructure construction,improve the capacity and level of health personnel,community health services is the future direction of development;C service is the community public health services need to strengthen and improve the goal.
