The Actual Evaluation And Cost Measuring Study Of Urban Community Public Health Services In Jiangxi Province

Objective:To estimate the current situation of the urban community public health services in Jiangxi province; To make a calculation of the factual cost and standard cost of community public health services and constitute the basic community public health service package; To analyse the factors which influence the development of community public health service and explore appropriate approaches to solve the problems.Methods:By means of stratified sampling, 10 three-stared community health service centers were sampled. Questionnaire survey, participatory observation and semi-structured interview were used during the survey. The quantitative data and qualitative data were collected. The quantitative data was input and settled by Excel2003 and then statistical analysis was made by SPSS11.5.Results:Firstly, on average, the development rate of public health service of the surveyed institutions is 77.42 percent, and the accomplishment rate is 64.31%.Secondly, as for the principals of surveyed institutions, the following four items are most important, they are register and administration of vaccination, vaccination and complementary vaccination, imperative inoculation, reporting the doubtful reaction of vaccination. The following four items were thought to be completed with more ability;They are register and changing diagnosis and treat of the doubtful lungers, measuring blood pressure, measuring blood sugar, the statistical report of vaccination.Thirdly, in Jiangxi province, the factual cost of community public health services is about 11.85 yuan every year for one person. The compensation money given by government is 11 yuan every year for one person. This two numbers are very approximate. The standard cost of community public health services is about 19.65 yuan every year for one person by measuring standard cost, while it is 21.05 by measuring complete manpower cost.Fourthly, according to the service standard and the compositor of importance and ability, there are seven big items and thirty six little items can be completed within the compensating fees given by government with a standard of 11 yuan every year for one person, and then the basic community public health service package is constituted.Conclusions:Firstly, the pattern of community public health services of Jiangxi province is formed elementarily.Secondly, according to the service standard, 11 yuan every year for one person is enough to complete the services in the basic community public health service package. If the government adds the compensating fees, some other items can be added to the basic community public health service package.Thirdly, in order to distribute the compensation money reasonably, scientific performance merit system needs to be constructed. Fourthly, it is time to establish a community health services system which is led by government, supervised by the health department and managed cooperatively by the correlative departments.
