| ObjectiveTo observe the clinical curative effect of Tang Shen Kang Yin in the treatment of patients with early diabetic nephropathy and the influence on serum VEGF expression levels,to evaluate the clinical efficacy,and to speculate the possible mechanism of Tang Shen Kang Yin.Methods80 patients of early diabetic nephropathy with qi and yin deficiency with blood stasis diagnosed in endocrine clinic of First Affiliated Hospital Heilongjiang University Of Chinese Medicine from January 2016 to January 2017 were selected as a subject of study,they were divided into treatment group and control group randomly,each group had 40 cases.All patients received basic treatment,including diabetes education,diet therapy,exercise therapy,subcutaneous injection of insulin to control blood glucose and irbesartan 75mg once a day orally,Tang Shen Kang Yin was added in the treatment group.The clinical observation lasted for 8 weeks,to observe the change of patients’ UAER,serum VEGF,blood glucose,blood lipids and TCM syndrome integral.Results1.After 8 weeks of treatment,the improvement of TCM syndrome score in the treatment group was significantly better than that in the control group(P<0.05).The total effective rate of TCM syndromes in treatment group and control group was 82.0%and 36.8%,The efficacy of TCM syndromes in the treatment group was remarkably better than that in the control group(P<0.05).2.After 8 weeks of treatment,the total effective rate of treatment group and control group was 87.2%and 55.3%,and the clinical efficacy of the treatment group was superior to the control group(P<0.05).3.After 8 weeks of treatment,the related indexes of blood lipid of two groups were improved before treatment(P<0.05),the difference between the two groups showed that the treatment group improved TC,TG,LDL-C,HDL-C levels were significantly better than the control group(P<0.05).4.After 8 weeks of treatment,UAER in both groups was significantly lower than that before treatment(P<0.05).The difference between the two groups showed that the treatment group decreased UAER significantly better than the control group(P<0.05).5.After 8 weeks of treatment,VEGF in both groups was significantly lower than that before treatment(P<0.05).The difference between the two groups showed that the treatment group decreased VEGF significantly better than the control group(P<0.05).Conclusion.1.Tang Shen Kang Yin can effectively reduce the level of UAER in patients with early diabetic nephropathy,thus relieve the grade of renel damagement.2.Tang Shen Kang Yin can improve the clinical symptom of TCM in patients with early diabetic nephropathy with Qi and Yin deficiency with blood stasis.3.Tang Shen Kang Yin can reduce the level of TC,TG and LDL-C,can elevate the role of HDL-C in patients,which can improve the lipid metabolism disorder.4.Tang Shen Kang Yin may be delayed the progression of DN by decreasing the expression of VEGF in serum. |
