Clinical Efficacy Of Tang Shen Kang Yin In Treating Early Diabetic Naphropathy And The Influence Of Col-Ⅳ、LN

Objective:To observe clinical curative effect of Tang Shen Kang Yin in treating early diabetic nephropathy(DN)with two deficiency of qi and yin with blood stasis type,and observe its influence on patients serum Col-IV、LN,and to explore its mechanism for the treatment of early DN may be.Methods:Select 80 patients who have got DN in accordance with clinical inclusion criteria,randomly divided into two groups(control group,treatment group),40 cases in each one.The control group were given basic treatment:including strengthen diabetes education,diet structure adjustment,setting individualized exercise plan,control blood glucose,blood pressure,control urinary protein.The treatment group take Tang Shen Kang Yin additionally on the foundation treatment,two groups were treated by a course of 8 weeks.Observe and compare the change of indices,such as TCM syndrome integral,TCM single symptom integral,urinary albumin excretion rate(UAER),blood fat,serum Col-IV,LN,etc.,before and after treatment of two groups,and compare the total effective rate of the two groups,furthermore,to statistical analysis on it.Results:1.At the end of the course,compare the total effective rate of the two groups:control group and treatment group were 87.2%and 55.3%respectively,with a significant statistical significance(P<0.05).The curative effect of the treatment group is superior to the control group.2.At the end of the course,there is a decrease in the TCM syndrome integral of the two groups compared before the treatment and the TCM syndrome curative effect of treatment group effective rate was 82.0%,the control group was 36.8%.It means to have a significant difference between the two groups(P<0.05).It states the improvement about the TCM syndrome of the early DN patients with two deficiency of qi and Yin with blood stasis type is superior to the control group.3.At the end of the course,the UAER value of the treatment group declined obviously by comparison with untreated(P<0.05).There is statistical significance when it was compared with the treated UAER value of the control group(P<0.05).4.After 8 weeks,the serum Col-Ⅳ and LN levels of the two groups are both decreased,comparing each with the untreated state of themselves,with statistically significant(P<0.05).The serum Col-IV and LN levels of treatment group significantly declined better than the control group(P<0.05).Tang Shen Kang Yin can reduce the serum Col-Ⅳ and LN of the early DN.Conclusion:1.Tang Shen Kang Yin can reduce the TCM syndrome integral of the early DN patients with two deficiency of qi and Yin with blood stasis type.2.Tang Shen Kang Yin can improve the clinical symptoms of TCM,such as languid,dry throat and mouth,dizziness and tinnitus,soreness and weakness of waist and knees,spontaneous sweating and night sweat,eat much and strave easily,thirst with desire for drinks,palpitation and insomnia,dysphoria in chestpalms-soles,constipa-tion and urine yellow.3.Tang Shen Kang Yin can reduce UAER of early DN patients,thus relieve the grade of renal damagement.4.Tang Shen Kang Yin can improve blood fat(TG.TC.LDL-C、HDL-C)related index,5.Tang Shen Kang Yin maybe slow the progress of early DN through lowering the level of serum Col-Ⅳ and LN.
