The incidence of Nonalcoholic fatty liver disease increased rapidly at present,but there have no effective medicine with significant evidence,while traditional Chinese medicine show great advantage in treatment of NAFLD.Objective:1.To systematically summarize the syndrome differentiation and treatment thought of professor Xue Boyu in treating Nonalcoholic fatty liver disease(NAFLD).2.To explore the effects of Modified Zexie Decoction on treatment of non-alcoholic fatty liver disease in rat.Method:1.Data processing platform Medcase V3.2,which is also an data recording and mining system,was applied to perform frequency and association rule analysis on the typical cases of Nonalcoholic fatty liver disease treated by professor,by mining data on symptoms,syndromes,pathogenesis and therapeutic principle.2.NAFLD rat model was established by high fat feeding(10%or 45%high fat feed)and divided into 4 groups:Control group,Model group,Modified Zexie Decoction group and Polyene Phosphatidyl choline group.3.Rats in different groups were given different doses of medication or placebo by weight.4.After infusing four weeks,serum samples were collected to test TC、TG、ALT、AST、HDL-C、LDL-C、GLU.5.HE staining was used to obseve the liver histological changes,and Immunohistochemistry technique was used to check the expression of Toll like recepyor 4.6.Westem-blotting was used to detect the expression of TLR4(Toll like receptor 4),MyD88(Myeloid differentiation factor 88),NF-κB P65(Nuclear factor-kappa B p65),P-P65(Phosphorylated P65),P38 MAPK(P38 Mitogen activated protein kinase),P-P38(Phosphorylated P38),JNK(c-Jun-NH2-terminal kinase),Caspase-1(Cysteinyl aspartate-specificproteases-1)in liver tissue.Result:1.There were 73 cases of NAFLD included in this study.The most common Clinical symptom lack of power.Damp-heat accumulat,liver-stagnation and spleen-deficiency,the deficiency of liver and kidney were the most frequent pathogenesis.Clearing away heat and eliminating dampness,soothing liver,strengthening spleen and tonifying yin were important therapeutic principles.The first five commonly used herbs were Rhizoma Atractylodis Macrocephalae,Sedum Sarmentosum Bunge,Red Peony Roots,White Peony Roots and Costus Root.2.After 4 weeks of treatment,Modified Zexie Decoction and Polyene Phosphatidyl choline can reduce the AST、ALT、TG、TC、LDL-C significantly(AST、TG P<0.05;ALT、TC、LDL-C P<0.01),and even GLUwas also reduced,HDL-C was up-regulated,but there is no significant difference among 4 groups.3.Compared with the control group,proteins in model group were incressed(TLR4 P<0.05,others P<0.01);and proteins in decoction group were decreased compared to model group(MyD88、P38 MAPK P<0.05,others P<0.01).The expression of TLR4 shows the same result in IHC.Conclusion:1.Professor Xue Boyu emphasized the the analysis of Phlegm dampness and blood stasis on the pathogenesis,So the common therapies for NAFLD were focused on activating spleen,regulating liver and tonifying the kidney.2.Modified Zexie Decoction could inhibit the liver inflammation and alleviate liver damage.The mechanism may be related to inhibiting the expression of protein in TLR4/NF-κB and MAPK signal pathway.
