| Objective:Analysis of OSAHS combined adenoid hypertrophy in older children after orthodontics treatment 9 months of upper airway form and characteristics of the interna-l air flow dynamics,to evaluate the effect of orthodontics for these patients.Methods:Take a 11-year-old male child with OSAHS and adenoid hypertrophy as an example,and collect his upper respiratory tract CT medical images of before and after nine months of Orthodontics treatment.First,to import the above CT data into the soft-ware and reconstruct 3-dimensional finite element models sing the method of surf-acerending.Further smoothing processing of the above model,grid structure,surface fitting and meshing,etc.Then modify adenoidectomy part information,to simulate aden-oidectomy,further to reconstructing the upper-airway model and post-processing in the same way.Then put the orthodontics treatment before and after orthodontics treat-ment,further adenoidectomy computer fluid dynamics calculation results were compa-red with theresult after orthodontics treatment.Results:(1)According to the upper-airway model before the orthodontics treatment,the main obstruction in children with planein adenoidectomy area,from palatophary-ngeal to the oropharynx back.(2)After orthodontics treatment,the pharyngeal cavity volume increased to 12.2 cubic centimeter by 22.5 cubic centimeter before treatment,on airway minimum cros-s-sectional area increase from 154.8 square milimeter to 224.28 square milimeter,the tongue and epiglottis widened the most narrow part diameter from 14.96 milimeter to 16.26 milimeter;(3)For all models in my experiment,the total pressure drop were significantly reduced from the apex nasi to oroph-arynx,inspiratory phase total differential pressure,fell to 176.0pa from 155.3pa befor the treatment,postoperative reduced to 34.8pa.The lowest airway pressur and the largest pharynx velocity are all located in the adenoidectomy area.After orthodontics treatment,the negative pressur from-41.0pa to-21.3 Pa,post-operative adenoid body into-10.7pa.Tthe air flow rate from 15.0m/s to 10.5m/s,and the smaller pharyngeal maximum velocity area of area,nose pharynx ministry has no obvious chang.Postoperative adenoid the maximum velocity in the region reduced by 6.09m/s and high velocity area almost disappear.Conclution:(1)Using surface reconstruction and 3-D finite element analysis method was established to older children before and after orthodontics treatment on airway flo-w field numerical models,based on this platform,the children OSAHS airflow field analysis and evaluation of orthodontics treatment effect of OSAHS in child-ren.(2)OA can expand up3 to the glossopharyngeal airway volue and lower airway narrow area of negetive pressure and pharynx resistance,can be used to treat the children with OSAHS blocking plane located in the area.(3)OSAHS in children combined with adenoid hypertrophy,after Orthodontics further line method are better than the effect of adenoidectomy orthodontics treatment,theref-ore,the combination of adenoid hypertrophy in children OSAHS after Orthodontics treatment,suggest early line of adenoidectomy. |
