Objective To assess noninvasive positive airway pressure (NIPAP) compliance and factors associated with NIPAP compliance among patients with obstructive sleep apnea hypopnea syndrome(OSAHS).Method In November 2013 to October 2015 to the first affiliated hospital of kunming medical university respiratory medicine measure and the cadre ward sleep measure and treatment center, and the second affiliated hospital of kunming medical university sleep center,and the yunnan province people's hospital sleep center line agencies PSG examination confirmed the diagnosis and treatment of OSAHS patients, and has set up a wearing a respirator treatment, to the first affiliated hospital of kunming medical university after respiratory medicine clinics and voluntarily accept the investigation of follow-up of 69 cases of patients, all patients by PSG diagnosed with moderately severe OSAHS patients. Read patients noninvasive ventilator storage memory chip, the usage of data to check the patients decide its compliance and record the average AHI index after treatment, using NIPPV objectively for> 4 h/d and at least 70% of the nights per week to determine compliance.33 patients compliance (Application group),36 patients noncompliance (Abandoned group).Record the patient's general situation and PSG monitoring data.To Make compliance questionnaire and through the way of outpatient follow-up or telephone return visit to questionnaire survey was conducted on 69 patients. Comparative the Application group and the Abandoned group related factors between the two groups of patients. we analyse the influencing factors and the compliance in patients with noninvasive positive airway pressure in obstructive sleep apnea hypopnea syndrome.Result Investigation of 69 cases, compliance for 33 cases, noncompliance for 36 cases.the compliance in patients is 47.8%. There was no statistically significant difference between the Application group and the Abandoned group in Age, gender, BMI, health education for patients, total treatment time, and different modes of breathing machine, the types of mask, mask humidifier application, system complication, history of chronic airway disease, solitary, upper airway surgery, confirmed to wear respirator treatment time interval,P>0.05. There was statistically significant difference between the Application group and the Abandoned group in initial AHI index,the initial LSPO2, AHI index after treatment, patients'families on health education, appointment, adverse reactions, bad habits of life, is not convenient to wear NIPPV, night insomnia,P< 0.05. With noninvasive positive airway pressure (NIPAP) treatment adherence of good and bad as dependent variable Logistic stepwise regression analysis shows that the influence factors of NIPPV treatment adherence Including the initial AHI index (P= 0.003), it is not convenient for wearing situation (P= 0.011), the bad habits of life (P= 0.002), night insomnia (P= 0.03), its the OR values were 0.959 (95% CI 0.932 0.986),5.269 (1.465 18.947) and 1.465 (1.921 20.982),4.127 (1.151 14.747).Conclusion 1. Application of noninvasive positive airway pressure (Nipap) breathing machine of OSAHS patients Compliance is 47.8%.2. The nitial AHI index is higher in patients with noninvasive positive airway pressure (nipap) treatment adherence, the better.3. Influence of treatment adherence for patients with OSAHS risk factors is have bad habits of life, not convenient to wear, amd insomnia at night.
