| Objects:.To investigate the risk factors of insulin resistance and its effects on blood pressure among young college students.Methods: 389 volunteers(involve 332 of male,57 of female)from the university divided into three groups according to homeostasis model assessment insulin resistance(HOMA-IR)and classified by third tertile method: low-IR(130,≤2.44),mid-IR(129,2.44-3.04),high-IR(130,≥3.04).Height,weight systolic blood pressure(SBP)and diastolic blood pressure(DBP)were measured,body mass index(BMI)and mean arterial pressure(MAP)calculated by corresponding formula.Serum triglyceride,cholesterol,high-density lipoprotein(HDL-C),low-density lipoprotein(LDLC),fasting glucose,fasting insulin,nitric oxide(NO),homocysteine(HCY)detected respectively.All data was performed using SPSS19.0 software including c2test,one-way analysis of variance,Pearson correlation analysis and Multiple stepwise regression analysis model methods.P < 0.05 was considered significant difference.Results: 1.General datas:(1)There was difference in three groups(P<0.05),The number of men was higher in high-IR and mid-IR groups than in low-IR group(87.6% VS 76.9%,P<0.05;91.5% VS 76.9%,P <0.05),however,There was no difference between mid-HOMA-IR and high-HOMA-IR group(P >0.05);(2)The weight and BMI increased with HOMA-IR,weight and BMI higher high-HOMA-IRgroup than mid-HOMA-IR group;higher in mid-HOMA-IR group than low-HOMA-IR group;(3)There were no differences of age and height in three groups(P >0.05).2.Blood fat:There were significant differences of triglyceride,HDL-C.(1)Triglyceride:1.16±0.73 mmol/l、1.3±0.65 mmol/l、1.58±0.93 mmol/l(P <0.05).(2)HDL-C :58.48±32.68 mg/dl、46.94±28.81 mg/dl;HOMA-IR are correlated with triglyceride and HDL-C(r:0.243、-0.151,P <0.05);(3)There were no differences of cholesterol and LDL-C in three groups(P >0.05);3.Fasting blood glucose and fasting blood insulin:There were significant differences of fasting blood glucose and fasting blood insulin.(1)Blood glucose:4.32±0.41mmol/l、4.64±0.37mmol/l、5.04±0.56mmol/l(P <0.01),r=0.628(2)insulin:10.5±1.49 m U/l、13.36±1.15 m U/l、16.16±1.85 m U/l,r=0.897;4.NO、NOs: There were significant differences of NO、NOs.(1)NO:88.04±17.61umol/l、87.48±18.62 umol/l、76.15±23.89 umol/l(P <0.01)(2)NOs :9.58±1.33 umol/l、9.11±1.65 umol/l、8.87±2.53 umol/l;HOMA-IR are negatively correlated with NO、NOs(r:0.243、-0.151,p<0.05)5.HCY:5.25±1.13umol/l、5.31±1.26 umol/l、7.14±1.87 umol/l respectively in each group.HCY was higher in mid-IR and high-IR group than low-IR group(9.11±1.65ug/ml VS 9.58±1.33 ug/ml P <0.05;8.87±2.53 ug/ml VS 9.58±1.33 ug/ml,P <0.05),however,There were no differences of between mid-HOMA-IR and high-HOMA-IR(P >0.05).HOMA-IR are positively correlated with HCY(r:0.138,P <0.05)6.Blood pressure:With the increase of HOMA-IR,peripheral systolic blood pressure,diastolic blood pressure,mean arterial pressure are gradually increased.According to HOMA-IR third tertilemethod,SBP,DBP,MAP in low-IR are118.35±12.76 mm Hg,75.72±9.35 mm Hg,86.51±10.15 mm Hg respect,130.62±14.31 mm Hg,83.19±9.19 mm Hg,95.38±10.38 mm Hg in high-IR.7.Multiple regression analysis shows that insulin resistance are mainly associated with fasting blood glucose,homocysteine(HCY).low-density lipoprotein(LDL-C),weight,nitric oxide(NO)(p<0.05)8.Multiple regression analysis shows that the peripheral systolic blood pressures are mainly associated with weight,cholesterol,blood glucose and homocysteine(P<0.05),peripheral diastolic blood pressures are mainly associated with weight,cholesterol,blood glucose and HOMA-IR(P <0.05),mean arterial pressure diastolic blood pressures are mainly associated with weight,cholesterol,blood glucose and homocysteine(P <0.05).Peripheral diastolic blood pressures are increased with depth of insulin resistance degree,after adjusting for age,sex,weight,BMI,LDL-C,NO,blood glucose and other factors(β=0.136,P=0.049).Conclusions: 1.There were difference of gender distribution in three groups in young college students;HOMA-IR is associated with weight and BMI;2.HOMA-IR is associated with serum triglyceride,LDL-C,HDL-C,blood glucose,insulin,NO,NOs,and homocysteine,but There were no differences of cholesterol in young college students;3.The growth of HOMA-IR is associated with high peripheral diastolic blood pressure.The high insulin resistance is a risk factor of hypertension in young college students,early detection and improving life style may avoid emergence and progress in related diseases. |
