ObjectiveTo study the knowledge-attitude-practice of caregivers in institutions of elder care in Zhejiang province in order to understand the current situation in the three aspects of the caregivers staff,analysis the three factors of knowledge,attitude and behavior of caregivers staff,provide suggestions for the improvement of the service quality of caregivers staff.MethodsThis study mainly through the following three methods:firstly,the literature review.Collected the information from Wanfang,CNKI and VIP Chinese database,from PubMed,Web of Science and other foreign database to understand tne situation and age status of domestic and foreign caregivers training system etc..Secondly,the questionnaire survey.A cross-sectional survey was conducted.From April to June 2016,we visited 10 institutions of elder care in Xihu district,Hangzhou and Lanxi County,Jinhua,and interviewed totally 84 caregivers face-to-face with the knowledge-attitude-practice questionnaires.Thirdly,the statistical analysis.The data collected were analyzed by rank correlation,stepwise regression and structural equation modeling.ResultsIn our study,we.found that the oldest caregiver was 72 years and the youngest was 23 and the average age of caregiver was 50.77 years.There were 78 female caregivers and only 6 male caregivers interviewed.In terms of their educational level,the overall level of caregivers education was not high and the level of education was mainly concentrated in the junior high school and below,of which there were 21 caregivers were illiteracy,accounting for 25%;by primary school had a population of 20,accounting for 23.8%.In terms of monthly income,the income of caregivers is not high,mainly concentrated in 3001—4000 yuan,a total of 21 people,accounting for 53.6%.We also found that their work hours were long,work intensity was large,work content was complex.The average daily working time was 14.96 ± 5.77 hours,the average caregivers need to take care of 7.96 ± 3.94 elderly,and their main working content was that keep the elderly living environment clean,help the elderly living,take responsible for the health of the elderly.There were 55 caregivers without pre-work training in all,accounting for 65.5%.After working in institutions,most of caregivers staff can receive different types of nursing training,most of them can received 2-4 times training,accounting for 45.2%.Followed by 5-8 times training.There were also 14 caregivers has not been accepted any types of nursing training.Their knowledge score of the maximum value is 23,the minimum value is 6 and the average score was 17.11,the standard deviation is 3.34;attitude score maximum value is 60,the minimum value is 38,the average score was 48.15,the standard deviation is 3.711;the practice score maximum value is 7,the minimum value is 1,the average score was 5.58 the standard deviation is 1.355.The regression analysis showed that the level of education,work experience,pre-work training had a positive effect on knowledge;pre-work training,monthly income had a positive effect on attitude,age had a negative impact on attitude;the times of training,education and working years had a positive impact on practice.Further structural equation modeling was used to find out thehow nursing knowledge impacted on nursing practice.ConclusionOur study found that the age of caregivers is older and the level of relative nuring knowledge is not high.And we highlight the importance of social and training variables in improving the elder care quality of caregivers.In order to improve the nursing quality of caregivers,we consider more public resources are needed to support and equip the caregivers,and to provide training chances for them,such as improve the nursing personnel training system,standardize the caregivers occupation team,improve the salary and welfare system for caregivers and so on.
