The Literature Of TCM Syndrome Type Of Burns And Evidence-based Medical Research

Objective: Through literature retrieval and evidence-based analysis,combing literatures about the rule of burns of traditional Chinese medicine,inducing the literatures about TCM syndrome types of burns which were often appeared.The objection is to standardize traditional Chinese medicine syndrome differentiation of burns and to provide evidence-based study basis of TCM syndrome differentiation and treatments of surgery of traditional Chinese medicine clinical diagnosis and treatment guidelines of burns which were host by my mentor.Methods: This study based on the literature research and evidence-based science principles and method,by a set of strict and normative retrieval strategy retrieval the modern literatures about burns of TCM syndrome differentiation and treatments which were published at home and abroad in nearly 26 years,establishing the literature,evaluating and classifying the literatures in this database,through according to the inclusion criteria and excluding standard by three filter to determine which literatures can be inclued.Getting these literatures of TCM syndrome types and related drugs intoliterature of TCM syndrome type and related drugs into factor analysis,principal component analysis and cluster analysis,founding the regularity of of burns,inducing the available in the literature about a common TCM syndrome types of burns.Studying the type of syndrome which was identified from literature research,combined with 200 clinical cases of clinical evaluation,and then to increase and delete the type of syndrome which was identified from literature research according to the clinical practice.Lastly,getting the type of syndrome which was after clinical consistency evaluated into the expert questionnaire by using the Delphi method.Through statistical analysis of the result of the questionnaire to determine the syndrome surgery stype of burns that should be adopted in the traditional Chinese medicine(TCM)clinical diagnosis and treatment guidelines(revised).Conclusion: Choicing in 89 published papers,related to burn a total of s5883 cases in these literatures and cases for information collection.The result wa collected 61 type of syndrome,83 kind of taken orally Chinese traditional medicine and 15 type of effective certificate.To score into literatures by using improved Jadad scalean,with an average of 2.8651 points.To score into literatures by using traditional medical evidence classification standard of Liu-Jiangping who were coming from Beijing university of Chinese medicine EBM center.It is concluded that grade?level with 0 references,grade ? level with 2 references(4.49%),grade ? with 59 references(64.04%),grade ? level with 18 references(31.46%).After requency analysis of TCM syndrome types of burns,we concluded that it has 15 type of syndrome,and the frequency top six evidence type in the order were syndrome of thermal injury Ying-Wei,syndrome of fire poison body fluid,syndrome of fire poison invagination,qi-blood deficiency and syndrome of YinShang and spleen deficiency,after do a clinical consistency evaluation between the six type of syndromes and 200 cases provided by a number of hospitals,combine syndrome of fire poison body fluid and syndrome of poison flaming fire into syndrome of fire poison body fluid,increase a syndrome of thermal injury Ying-Wei,sccording to thermal injury Ying-Wei,syndrome of fire poison body fluid,syndrome of fire poison invagination,qi-blood deficient and syndrome of YinShang and spleen deficient to set the Delphi expert questionnaire,expert consensus degree is more than 80%.After doing frequency analysis with the common symptom in burn in the literature,we inductive to 63 symptoms,doing component analysis with these 63 symptoms,the statistics table suggests five before the cumulative contribution rate of more than 94% of the principal component,the rotating factor loading matrix B based on hierarchical clustering method divided these symptoms into five major groups.After doing frequency analysis with commonly used internal and traditional Chinese medicine(TCM),we inductive to 83 kind of commonly used internal and traditional Chinese medicine(TCM),and then doing component analysis with these 83 kind of commonly used internal and traditional Chinese medicine(TCM),the results suggest that before 6 principal components of the cumulative contribution rate of more than 94,again on the factor loading matrix B based on hierarchical clustering method,the traditional Chinese medicine(TCM)is divided into nine major groups,does not quite agree with clinical,but it in terms of basic burns treatment from the drug efficacy that clearing away heat and toxic material in the early,supplementing qi and restoring Yang in the medium term and invigorating spleen and stomach,replenishing qi and blood in the later.Results:1.Research shows that the highest frequency types name were fire poison damage to body fluid syndrome,poison flaming fire certificate syndrome,Yin deficiency and Yang to take off syndrome,fire poison invagination syndrome,Yin Shang pixu(spleen deficient)and impairment of both qi and blood syndrome,among them fire poison damage to body fluid syndrome and poison flaming fire certificate can be due to the same principal components and groups,the two can be combined into type.2.No matter it's for literature,burn symptoms of frequency analysis,principal component analysis or clinical study showed that mild burns who often only local clinical symptoms with the lack of systemic symptom should be a relevant burn light disease pathogenesis symptoms name with the system.Combined with the related materials and books provided by the basic pathogenesis of burns,burn camp guard card is the most appropriate name.3.The conclusion of literature research and clinical evaluation of consistency were that burn injury Ying-Wei syndrome,poison flaming fire certificate syndrome,YinShang Yang to take off the certificate syndrome,fire poison invagination syndrome,YinShang pixu(spleen deficient)and impairment of both qi and blood syndrome can be used as the guide in evidence.This conclusion has been through the Delphi method to investigate and experts have reached a consensus.
