| Objective:Compared the clinical curative effect of the conventional scheme with zuogui pill combined with the conventional scheme in treatment of osteoporosis(kidney essence deficiency type) after the total hip arthroplasty by the analysis of the clinical trials research to provide a new thought and method, and verify the effectiveness of traditional Chinese medicine and the superiority, and restrain the loss of bone after the total hip arthroplasty, meanwhile, by improving the quality and strength of bone and reducing the symptoms of pain and the incidence of fracture to optimize the quality of patient’s life after the THA.Methods : Selecting during September 2014 to December 2015 in orthopaedic YangMei Group General Hospital of 56 patients which is going to take the THA accompaning by osteoporosis(kidney essence deficiency type) as the research objects those were randomly divided into treatment group and the control group(n = 28) under general anesthesia by the same group of researchers to patients, laterally taking the“Hardinge” which is “Lateral approach of THA” as the standard operative approach, and then the type of prosthesis is bone cement. During the operation monitoring patients ’blood pressure and preventing crisis of “bone cement” after the THA. Postoperative routine to prevent infection, anticoagulation, functional exercise treatment, two groups of patients with postoperative analgesia pump should be used in 48 hours to relieve the pain.In order to avoid the influence of anesthesia and postoperative systemic stress reaction,research start from 3 days postoperatively. Treatment group: given zuogui pill combined conventional anti osteoporosis scheme for treatment; Control group: routine clinical administration of anti-osteoporosis treatment process. During the treatment, the two groups of patients were properly carried out daily sunlight, the body promoting active vitamin D3 generation, in conjunction with dietary modification, rehabilitation after surgery, 6 months,whichever effects were compared. At the end of the treatment, respectively, testing two groups of patients in the clinical signs and symptoms and bone density, before and after treatment, including serum osteocalcin(BGP), 1,25-(OH) 2D3, urinary Ca, HOP, Ca / cr,HOP / changes cr of efficacy evaluation. General observations before and after surgery include postoperative hospital stay, infection, healing time, before and after surgery Harriship score and postoperative complication rate. Combined with the actual situation by using SPSS 23.0 statistical software measuring the data from clinical trials, and then drawing the relevant conclusions from the statistical analysis.Results : 1. The subjects were followed, and the clinical trial patients, the total effective rate was 96.4%, the control group was 82.1%, P <0.05, show the efficacy of the experimental group than the control group, we can see with conventional anti-bone osteoporosis treatment, compared Zuogui pill combined with the anti-osteoporosis treatment routine program for kidney essence deficiency osteoporosis effect is more obvious; 2. two groups of patients with pain scores were compared, the results showed the two groups pain points are before and after treatment in patients with a significant difference, and the experimental group compared with the control group, P <0.05,significantly different results, namely Zuogui pill combined with the conventional anti-osteoporosis treatment program to improve the degree of pain symptoms are better than anti-osteoporosis treatment; 3. two groups of subjects Syndrome score were compared with each other, and the results are shown that before and after treatment in both groups symptom score were more significant differences, and the test group and control group two compared between groups, P <0.05, there were significant differences in the experimental group symptom score improved significantly, that compared with the control group,Zuoguiwan combined with the conventional anti-osteoporosis treatment program for improving the bone caused by osteoporosis TCM syndrome are more obvious; 4. the BMD was measured before and after treatment in two groups were compared, P <0.05, there is a significant difference, and the difference between the two groups was statistically significant, P <0.05. 5. after the treatment, two group of patients with the data of serum osteocalcin(BGP), calcium, 1,25-(OH) 2D3, Ca / cr, HOP / cr, HOP, are improved than before, while among several experimental group and control group indicators compared, P<0.05, the difference was statistically significant, the effect of improving the test group is superior to the control group. 6. The two groups were in postoperative hospital stay,infection, healing time, postoperative Harris hip score and postoperative complication rate were compared with each other, the difference was statistically significant.Conclusion : Zuogui pill combined with the conventional scheme in treatment of osteoporosis(kidney essence deficiency type) after the total hip arthroplasty is satisfactory,by reducing postoperative complications, and effectively relieve their symptoms, while improving bone mass in patients with traditional Chinese medicine loose syndrome, and reducing postoperative pain, prevention of osteoporosis and other aspects have a significant effect in a long term can be achieved effectively. Preventing from the postoperative prosthesis loosening subsidence, periprosthetic fracture, a series of postoperative pain and the osteoporosis related complications. |
