Clinical Study On Baihui Acupoint Acupuncture Point Treatment Intervention On Depression After Cerebral Infarction

Purpose:To observe the Baihui acupoint acupuncture point treatment, namely to observe Baihui moxibustion in box, acupuncture plus point injection of Fleabane injection for PCID intervention on efficacy and safety; to observe the IL-6, TNF-α expression in patients’serum and probe its relationship with the PCID, and observe the Baihui acupoint acupuncture point treatment bringing the effects on IL-6、TNF-α.Method:Select 60 patients with PCID in Inpatient Department of Department of Neurology, Guangzhou hospital of TCM from April 2015 to February 2016, and bringing these 60 patients divide into treatment group and contrast group (each group have 30 patients) according to random number table. On the basis of cerebral infarction treatment, the contrast group patients each take Sertraline 50mg qd; the treatment group take Baihui moxibustion (qd), acupuncture (qd) and fleabane point injection (0.3ml qod). This treatment will last for 14 days. By comparing HAMD score, neurological deficit scores, Barthel Index score before and after treatment, to evaluate the efficacy of two treatment methods; by contrasting side effects of two groups to assess the safety of the two treatment; by observing the IL-6,TNF-α expression in patients’serum, to probe its relationship with the PCID and compare two treatments having effects on IL-6 or TNF-α.Result:1. Comparison of general clinical data:the patient’s sex, age, duration and type of TCM difference have no statistical significance before treatment; the date can be compared.2. Comparison of HAMD scores:the difference of patients’scores between the two groups have no statistical significance before treatment (P> 0.05), the date can be compared. After treatment, compare the patients in each group, HAMD scores all reduced, and the difference had statistical significance (P<0.01) by statistical analysis. Compare two groups, we find the difference have no statistical significance (P>0.05). The clinical results of two groups show:total effective rate of Baihui acupoint acupuncture point treatment group was 83.3%, and the total effective rate of sertraline group was 70%. The comparison of two groups’efficacy after treatment have statistical significance (P<0.05).3. Comparison of clinical neurological deficit scores:the difference of patients’scores between the two groups have no statistical significance before treatment (P> 0.05), the date can be compared. After treatment, compare the patients in each group, clinical neurological deficit scores all reduced, and the difference had statistical significance (P<0.01) by statistical analysis. Compare two groups, we find the difference have no statistical significance (P>0.05). The clinical results of two groups show:total effective rate of Baihui acupoint acupuncture point treatment group was 60%, and the total effective rate of sertraline group was 46.7%. The comparison of two groups’efficacy after treatment have no statistical significance (P>0.05).4. Comparison of Barthel Index scores:the difference of patients’scores between the two groups have no statistical significance before treatment (P >0.05), the date can be compared. After treatment, compare the patients in each group, Barthel Index scores all improved, and the difference had statistical significance (P<0.01) by statistical analysis. Compare two groups, we find the difference have no statistical significance (P>0.05).5. Comparison of side effects scale:the result of side effects scale after treatment show:scores between the two groups have significant difference (P <0.01), which the side effects of sertraline group score is higher.6. Analysis of laboratory parameters:the IL-6、TNF-α in serum of PCID patients all elevated, there was a significant difference (P<0.01) compared with the normal value. By comparing the correlation between IL-6, TNF-α and PCID, the result show:29.9% of HAMD depression scale score’s variation can be explained by changes of IL-6 value; and 10% can be explained by changes of TNF-α value. the difference of patients’ IL-6、TNF-α between the two groups have no statistical significance before treatment (P>0.05), the date can be compared. After treatment, the IL-6、TNF-α in serum of two group all reduced, but the comparison of two groups or patients in each group have no statistical significance (P>0.05).Conclusion:1. Baihui acupoint acupuncture point treatment and sertraline treatment both can significantly improve the patient’s degree of depression PCID, but Baihui acupoint acupuncture point treatment is better for fewer side effects. So for PCID treatment, Baihui acupoint acupuncture point treatment has a good efficacy and few side effects.2. Baihui acupoint acupuncture point treatment and sertraline treatment both can effectively improve the clinical neurological deficit scores and Barthel index score. In terms of improvement of neurological deficit, the effect of Baihui group is better. The result illustrate Baihui acupoint acupuncture point treatment and sertraline treatment can not only improve the patient’s depressive symptoms, but also promote the recovery of nerve function.3. IL-6, TNF-α in serum of two group patients all increased in varying degree, and IL-6, TNF-α both were positively correlated with the occurrence of PCID. Baihui acupoint comprehensive treatment and sertraline both can make IL-6, TNF-α value reduce after treatment, but the effects is not obvious.
