Objective: The clinical and pathological characteristics ofdifferent age groups of patients with colorectal cancer analyzing for improvingthe diagnosis and treatment of colorectal cancer, and improving the prognosisof patients with youth. Methods: The clinical and pathological data collected inour hospital from January2007to December2010for treatment of749cases ofcolorectal cancer patients.The patients were divided into three age groups,youth group (<35years old,48cases), middle-aged group (36-60year old,305cases), older group (>61years,396cases). A retrospective analysis wasadministrated for different age groups of patients and their clinical pathologicalfeatures. The clinical pathological features included incidence, gender, locationof tumor, time of onset of symptoms, the first symptoms, histological grade,gross type, histological type, staging, treatment, prognosis. Results: Thepopulations of colorectal cancer are older people, the patients of youth groupare only6.4%; Male patients of youth group are slightly higher than femalepatients, and there is no significant difference; Rectal cancer incidencegradually increased and colon cancer incidence gradually decline with ageincreasing.2weeks to2years of colorectal cancer is the most common time ofonset of symptoms, accounting for74.6%of all cases, the youth group,<2weeks were31.3%, youth group between the onset of symptoms in the elderlygroup and time were significantly different (P=0.000); Abdominal pain andblood in the stool are the common first symptom of youth group, and blood in the stool and change in bowel habits are the common first symptom in theelderly group,(P=0.049); Misdiagnosis rate in youth group was significantlyhigher than in the older group (P=0.000); Pathological tissue types: Adencarcinoma of the youth group of patients was43.8%, significantly higher thanin the older group, and various age group of patients with different pathologicaltypes were significantly different (P=0.000); There is no significantdifferences of protein expression of P53and Ki67in colorectal cancer tissueamong different age groups; Serum CEA levels was significantly higher inyouth group than that in the elderly group (P=0.000); Compared with themiddle-aged group, the clinical pathological stage of young group is mainly â…¢and â…£ (P=0.000), the histological grade of differentiation is mainlymoderated gland carcinoma and poorly differentiated Aden carcinoma (P=0.000), gross type with cauliflower, ulcerative based (P=0.004), a higherproportion of palliative surgery (P=0.000), poor prognosis, the survival rate islower than in the older group (P=0.000).Conclusion: There were significantdifferences in initial symptoms, histological type, stage, tumor differentiation,serum CEA level, the rate of misdiagnosis and prognosis of colorectal cancerpatients in different age groups, especially in young patients with atypicalclinical symptoms, misdiagnosed, diagnosed late, poor cancer celldifferentiation, high degree of malignancy, poor prognosis and should be highlyvalued; clinical work should strengthen the popularization of colorectal cancer,colorectal cancer patients with suspected intestinal symptoms implement ameans to check a smear of two principles, emphasis on long-term follow-up of high-risk populations and reasonable and effective review of diagnosis andtreatment, combined with the determination of serum CEA, and strive to earlydiagnosis and treatment, reduce misdiagnosis, thus improving the cure rate andsurvival of colorectal cancer, colorectal cancer to improve youth prognosis. |
