Objective:In practice,CK7 is positively expressed in some colorectal cancer(CRC),and its clinical significance is worth exploring.This study aimed to clarify the expression of CK7 in CRC,evaluate the correlation between clinicopathological parameters and patient prognosis,and attempt to clarify the prognostic significance of CK7 for CRC.Methods:A total of 1105 cases of colorectal cancer diagnosed by the Department of Pathology of the First Affiliated Hospital of Wannan Medical College from January 2018to December 2020 were collected,and all cases underwent radical resection.All cases were re-reviewed,wax blocks were selected to make tissue chips,and CK7 expression was detected by immunohistochemical staining.The Chi-square test was used to analyze the correlation between common clinicopathological parameters and positive expression of CK7,and P<0.05 was considered significant.The included samples were followed up to count the recurrence,death,survival and treatment of CRC patients.Univariate logistic regression analysis was performed on the data,and P<0.05 was considered significant.At the same time,the Kaplan-Meier method and log-rank test were used to perform survival analysis to analyze the influence of conventional pathological factors on the prognosis of CRC.Using the Cox regression model,the multifactorial factor is the independent influencing factor of the patient’s survival state.Results:CK7 was mainly expressed in the cytoplasm.With>10%tumor cell staining as positive expression,the positive expression rate of CK7 in colorectal cancer was 7.42%(82/1105).There was a significant difference in tumor lesion location between CK7+CRC and CK7-CRC(X~2=10.260,P=0.036).There was a significant difference in tumor differentiation between CK7+CRC and CK7-CRC(X~2=12.761,P<0.001).There was a significant difference in the degree of germination between CK7+and CK7-CRC tumors(X~2=14.798,P=0.001).Univariate logistic regression analysis showed that histology,degree of differentiation,vascular cancer thrombus,depth of invasion,distant metastasis,tumor recurrence and CK7 expression were risk factors affecting the prognosis of colorectal cancer patients.The Cox regression model was used for vascular cancer thrombus,depth of invasion,distant metastasis,TNM stage,tumor recurrence,and CK7expression as independent factors affecting the prognosis of CRC patients(P<0.05).The survival curve was plotted by the Kaplan-Meier method,and the statistical differences between the survival curves were analyzed by the Log-Rank test method.The analysis found that patients with CK7-positive CRC had a worse survival prognosis than those with CK7-negative CRC,and the difference was statistically significant(P<0.001).Patients with CRC with vascular cancer thrombosis,deeper invasion,distant metastasis,later TNM stage,and tumor recurrence have a poor prognosis(P<0.001).Conclusions:1.CK7 may be an independent factor in poor prognosis for colorectal cancer.2.CK7 positivity has a correlation with the tumor anatomical site,degree ofdifferentiation and tumor budding of colorectal cancer,CK7 is frequently expressed in ascending colons,poorly differentiated tumors,and tumor budding cells..
