Influence Of Noninvasive Ventilation On Body Mass Index, Serum Albumin And Immunoglobulin, Lymphocytes In Patients With COPD And Respiratory Failure

Objective:To explore influence of noninvasive ventilation on body mass index, serum albumin and immunoglobulin, lymphocytes in patients with COPD and respiratory failure.Methods:25 patients with COPD and respiratory failure who accepted noninvasive ventilation in hospital and continued noninvasive ventilation and oxygen therapy at home were as experimental group,27 patients with COPD and respiratory failure who accepted noninvasive ventilation in hospital and stopped noninvasive ventilation to continue oxygen therapy at home were as control group. Both experimental group and control group accepted the treatments for 15 hours a day, and observation time was 6 months, And two groups were given the same bronchial diastolic and phlegm. Observed two groups of patients with body mass index (BMI), serum total protein(STP), serum albumin(ALB) and globulin(GLB), concentration of IgM,IgA and IgG,peripheral blood lymphocyte count (TLC), Compared the change of the observational indexes in every group at discharge and 6 months after discharge,and compared the change of the observational indexes in two groups 6 months after discharge.ResuIts:(1)Arterial blood gas of experimental group conducted that there was insignificant difference among the patients at discharge and 6 months after discharge between the levels of PaO2(82±10.8)mmHg VS(80±6.9)mmHg and PaCO2 (42±4.6)mmHg VS (43±5.6) mmHg (all P>0.05).(2)In control group:the level of PaO2 about the patients at discharge was higer than that of 6 months after discharge(84±12.4)mmHg VS(62±8.4) mmHg, and the level of PaCO2 about the patients at discharge was lower than that of 6 months after discharge(41±3.9)mmHg VS(51±6.2) mmHg(all P<0.05).(3)Compared the experimental group in BMI(18.37±1.05) kg/m2 VS(19.62±1.12)kg/m2,STP(49.90±2.96)g/LVS(57.80±2.48)g/L,ALB(31.30±3.97)g/LVS(35.70±1. 70)g/L and GLB (18.20±3.65) g/L VS (23.10±4.04), IgG(10.36±1.80)g/L VS(12.46±1.29)g/L,IgA (2.02±0.22) g/L VS (2.31±0.14) g/L, TLC(0.85±0.39) x109/L VS (1.67±0.58) x109/L,the indexes of the patients 6 months after discharge were significantly higher than that at discharge(all P<0.05).(4)Compared the two groups in BMI(19.62±1.12)kg/m2VS(18.24±0.97)kg/m2, STP(57.80±2.48)g/L VS (51.09±2.54)g/L,ALB(35.70±1.70)g/L VS (31.27±1.49)g/L,GLB (23.10±4.04)g/LVS(19.72±3.01)g/L,IgG(12.46±1.29)g/LVS(10.58±1.26)g/L, IgA(2.31±0.14) g/L VS (2.04±0.18) g/L,TLC (1.67±0.58) x 109/L VS (0.92±0.40) x 109/L after 6 months,the indexes of experimental group were significantly higher than control group (all P<0.05).(5) The levels of BMI,STP:ALB,GLB,IgG,IgA and TLC were insignificant difference among the control group at discharge and 6 months after discharge(all P>0.05).Conclusions:Noninvasive ventilation can improve body mass index, serum albumin and immunoglobulin,lymphocytes of the patients with COPD and respiratory failure.
