Research On The Recipe Features And Relationship Between TCM Syndrome And Laboratory Parameters In146Cases Of Chronic Hepatitis B Patients

ObjectiveTo analyze the cases with chronic hepatitis B which involve the inpatients in the stomach-Branch of our hospital in three years, including TCM distribution, treatment by Chinese medicine and the relationship between various syndromes and laboratory parameters.MethodsWith the cases data collected in the stomach-Branch of our hospital in three years, according to the four diagnostic parameters of TCM, the syndrome types can be divided into five, including Damp heat, Stagnation and spleen deficiency, Liver-kidney yin deficiency, Blood stasis and Spleen-kidney yang deficience. Moreover, I induce the TCM distribution, treatment by Chinese medicine and assemble the correlative laboratory parameters. Finally, I use statistical software to analyze data.ResultsA total of146patients (117male and29female) are included in the study, whose diagnosis records are from Nov.2011to Mar.2014. Patients range in age from19to79years old, with a mean of49.5±4.5. Their average of course is7.67±2.65years.According to the standard of Dialectical type, the result is Damp heat(n=66), Stagnation and spleen deficiency(n=32), liver-kidney yin deficiency(n=15), Blood stasis (n=21) and spleen-kidney yang deficience (n=12).From the analysis, in respect of the laboratory parameters, such as ALT, AST and TB, the Damp heat patients are apparently higher than the liver-kidney yin deficiency,spleen-kidney yang-deficience and Blood stasis patients (p<0.05), but there is no difference between the Damp heat and Stagnation and spleen deficiency. And Stagnation and spleen deficiency is significantly higher than the Liver-kidney yin deficiency (p<0.05), however, both of them have no significant on TB, HBV-DNA levels(p>0.05). Compare Stagnation and spleen deficiency and Blood stasis with AST, ALT, HBV-DNA, the former is higher than the latter(p<0.05), but both of them make no difference on TB(p>0.05). Nevertheless, Stagnation and spleen deficiency is apparently higher than Spleen-kidney yang deficience in contrast to ALT, AST, TB and HBV-DNA(p<0.05). Contrarily, the Liver-kidney yin deficiency, the Blood stasis and the Spleen-kidney yang deficience make no difference in contrast to ALT, AST, TB and HBV-DNA(p<0.05).In terms of sexuality, the percentage of men(n=117) far outweigh women(n=29). Besides, the data of AST, ALT and HBV-DNA in men is significantly greater than those in women, but both of them have indifference on TB level.For the treatment by Chinese medicine, the most common of Chinese medicine in five types as follows. Damp heat type(n=66):Capillaris(60times), scutellaria(58times), gardenia(58times), poria(56times), alisma(54tim es), agastache(50times), perrin(48times), radix(40times), coix(38times) and rhubarb (28times);Stagnation and spleen deficiency(n=32):bupleurum (30times), codonopsis(30times), poria(28times), atractylodes (26times), astragalus (23times), white penoy root (23times), citrus aurantium (22times), woody (20times), angelica (18times) and jujube (16times);Liver-kidney yin deficiency(n=15):rehmannia(14times), wolfberry (14times), dogwood(13times), yam (12times), radix(11times), angelica (10times), dan (9times), alisma (9times), jujube (7times), poria (6times);BTood stasis(n=21):salvia (18times), moutan (17times), angelica (16times), TPG (16times), sweet (12times), Radix (12times), wood (10times), betel nut (8times), guizhi(8times), licorice (6times);Spleen-kidney yang deficience(n=12):rehmannia(10times), yam (10times), dogwood(10times), ginger (9times), cinnamon (8times), poria(8times), codonopsis(7times), astragalus(6times), monkshood(5times), lie orice (5times).ConelusionWe can find that most of TCM syndrome are the Damp heat in146cases of chronic hepatitis B patients and its quantity is66cases(45.2%), which is consider to be a correlation between the high temperature and rainy in south of the Five Ridges area and dietary habits. Therefore, in addition to symptomatic medicine in clinical treatment, we should use dampness heat drugs to treat this credit. Five syndrome types in AST, ALT, TB, HBV-DNA, Damp heat and stagnation of liver qi and spleen deficiency are obviously higher than the other three types(p<0.05).But there is no different between them. It is considered that both of them are at an early stage of chronic hepatitis B, which belong to immune clearance phase. At this point, the immune scavenging capacity is greater than the others, and when the immune mechanism starts to kill hepatitis virus, it can cause varying degrees of destruction of hepatocytes inevitably, so this causes that Damp heat and stagnation of liver qi and spleen deficiency are obviously higher than the other three types on ALT, AST. Moreover, in terms of the performance of TCM, the immune system of sthenic syndrome patient is stronger than asthenic syndrome patient.In the other way, the immune scavenging capacity of Damp heat and stagnation of liver qi and spleen deficiency are stronger than the other three, which confirms that sthenic and asthenic syndrome of Chinese medicine is scientific.On the comparison of men and women, we can discover the disadvantage elements like smoking, alcohol consumption, staying up late and work pressure in male group are worse than those in female group, which lead to the data of AST, ALT, TB, HBV-DNA are apparently higher than female.Finally, according to summary the characteristic treatment of five types, we can provide the clinical treatment of chronic hepatitis B with guidance basis.
