Chinese Medicine Syndrome Characteristic Research In Chronic Hepatitis C

Ojective:By collecting clinical cases, symptoms and laboratory test results, the relationships of chronic hepatitis C of TCM prescriptions, symptoms and laboratory tests are observed after analyzing them mutually, which is good for the treatment of chronic hepatitis C in providing a scientific basis, and is more conducive to the exchange of research and evaluation.Methods:All 100 patients in NanJing diagnosed as chronic hepatitis C patients are from April 2009~March 2010 University Affiliated Hospital Infection Section, People's Hospital of Jiang Su Province,and the second hospital infection ward. Clinical data are collected to creat databases including a brief medical history, symptoms, syndrome type, laboratory tests (such as liver function, HCV-RNA quantitative, B super-3) and so on. Infection routes, symptoms, characteristics of the Chinese medicine types of patients with CHC are analyzed by using SPSS13.0 statistical software, and the correlation of objective indicators and the Chinese medicine types will be researched after that.Results:1. It is observed the diverse ways of HCV infection in Nanjing,, including blood and blood products transfusion, drug abuse, iatrogenic transmission, sexual contact, close contact and so on. Blood and blood products transfusion accounts for 73%, which prompted that blood and blood products transfusion remains the most important means of infecting HCV in Nan Jing currently. The number of drug addicts infected with HCV, accounting for 10%, represents the second place in this survey, which indicates the number of HCV infection by drug abuse are stilll rising.2. In this survey it is collected 67 symptoms in all in CHC patients, symptoms of high frequency is fatigue (67%), flank pain (65%), dark urine (65%), irritability (46%),back andleg weakness (46%), heave a deep sigh (42%), bitter taste of mouth (41%), abdominal distention (41%), hydrodipsia (41%). while symptoms of lower frequency is edema (13%), chilly (12%), yellow body and eyes (11%), conjunctival congestion (11%), reheat (10%), few menstruation (10%), pale (9%), epigastric burning (9%), hectic fever (4%), cold hands and feet (4%).3. In this survey of 100 CHC patients, the type of damp-heat in the liver and cholecyst accounts for 38%, blood stasis type possesses 23%, type of deficiency of the Liver-yin and Kidney-yin accounts for 18%, stagnation of liver-QI with deficiency of the spleen type possesses 16%, and deficiency of Spleen-yang and Kidney-yang type accounts for 5%, The primary cause of excess type due to accumulation of damp-heat in the liver and cholecyst and blood stasis type, while deficiency type made up with deficiency of the Liver-yin and Kidney-yin type in the majority. Only a little type are classified to the type of deficiency of Spleen-yang and Kidney-yang.4. The levels of ALT and AST in group of stagnation of liver-QI with deficiency of the spleen in CHC patients were higher than the other four groups. While the levels of TB, DB, GGT and ALP analyzed between the two groups respectively showed no significant difference.5. Regression analysis shows that viral replication is almost close to the uniform distribution among different types, which is to say there is no direct correlation between Chinese medicine types and viral replication index in CHC patients.6. CHC is generally a long course disease, but Chinese medicine types associated with the course of the disease in inflammation stage are not evident.7. Order In accordance with the B-score from the heavy to light:deficiency of Spleen-yang and Kidney-yang type> type of deficiency of the Liver-yin and Kidney-yin> blood stasis type> stagnation of liver-QI with deficiency of the spleen type> the type of damp-heat in the liver and cholecyst. It is thought that patients in type of deficiency of Spleen-yang and Kidney-yang owe the most severe liver injury, while people of the type of damp-heat in the liver and cholecyst have the minimum liver damage, which is consistent with CHC development process ---from damp type to deficiency type through the blood stasis process.Conclusion:1. Blood and blood products transfusion remains the most important means of infecting HCV at current in Nan Jing.2. CHC is a long course disease, and its symptoms are more complicated than thought before. The symptoms of both type damp-heat in the liver and cholecyst and the stagnation of liver-QI with deficiency of the spleen type are more common, while less common symptoms are found in type of deficiency of Spleen-yang and Kidney-yang. 3. The primary cause of excess type in CHC patients due to accumulation of damp-heat in the liver and cholecyst and blood stasis type, while deficiency type made up with deficiency of the Liver-yin and Kidney-yin type in the majority. Only a little type are classified to the type of deficiency of Spleen-yang and Kidney-yang.4. The symptoms of patients with chronic hepatitis C, Chinese medicine types, some liver function index, and the damage of liver displayed in B ultrasonic are correlated to a certain extent. From the results of Clinical serology testing and B ultrasonic investigator can observe the characteristics of CHC in each stage dynamically, and review reciprocal transformation of different Chinese medicine types in them.
