Research On General Practitioners’ Job-transfer Train Present Situation Of Yunnan Province

Purpose This study is based on lots of literatures in China and abroad about general practitioner training or education, understand the family physicians’basic situation of domestic and foreign and related theory achievement of general practitioners training, and research the general practitioner training status of Yunnan Province, find out the main problems and influencing factors in the process of GPs training, on this basis, put forward scientific and reasonable measures for its improvement, improve the training level and quality, we hope to provide reference for the relevant departments in Yunnan province to further standardize the general practitioner training.Methods In this study social medicine, health service management, medical statistics theories were integrated used, combined qualitative methods and quantitative methods, research the present situation of Yunnan Province general practitioners training, the main methods included:(1) Literature review.(2) Quantitative research (questionnaire):purposive sampled6states to carry out the general practitioner training in Yunnan province, then171students participating in the2012training were randomly selected to be surveyed using a self-administered questionnaire, the unified questionnaires were surveyed on the spot by trained investigators and recovered when they finished.(3) Qualitative research (group discussion and individual in-depth interviews):convened training6to7students6bases to participate in training, general medical teacher3-4for group discussion. Individual in-depth interviews were conducted for a responsible person in each training base. Discussion and interviews were mainly for the talking object that there is general practitioner training problems and suggestions. Results(1) By the end of April,2014,the number of general practitioners of Yunnan Province has trained are only1980.(2)The quantitative results:general practitioner training time is1years, students think that the most ideal training time is average15.23months, about1/3of students think that the training time is short.66.08%of the participants satisfy with training content, by logistic regression analysis, the training content is affected by education, only45.21%of the students think that the training content meet the need of the actual work, most need to strengthen training content are general medical theory and clinical skills. Only53.80%of the participants satisfy with the training materials and "Examination Handbook";56.73%,58.48%,63.86%students satisfy with the theoretical training, clinical training, grass-roots practice training methods, students think the most suitable way of three training stage learning are distance education, clinical practice and basic practice.63.16%of students are satisfied with training management process, the whole training teaching process mainly is closely related to the training content clinical practice training method and grass-roots practice training methods. As of the end of2013the first general medicine training classes, Yunnan Province general practice teachers are918, but there are16.01%teachers are below attending physicians title, only13.18%grass-roots practice teachers in the total number,84.21%、78.36%students think that teachers are "good", the ability most teachers need to improve is the basic theory of family medicine. General practitioner training bases have24clinical bases, all are three grade general hospitals,16GPs clinical training bases with the support of central government finance is under construction, each clinical base correspond to2-3grass-roots practice bases,grass-roots practice bases are42; government subsidies for each student10000Yuan/person.(3) Qualitative results:after interviews, there are following problems:the first is contradictions between work and training, the second is GPS is not high profile, the third is students、teachers and the unit of work do not attach importance to training, the fourth is the supporting policies are imperfect, the fifth is lack of training funds, the last is training base lag behind hardware construction and so on.Conclusions 1.The number of general practitioner is short sevely, by the end of2015, from Yunnan province " twelfth five-year " health planning target4500are more than2500people;2.The training time is short; the contents of the training set reasonably, there is a gap between the training content and the actual need, the students hope to strengthen the general medical basic theory and clinical skills; students’the training materials’and "Examination Handbook’" satisfaction is not high; the students have lower satisfaction with theoretical training and clinical training methods; general practitioner training is generally good, affected by various factors such as the training content; the number of general medical teachers are not enough, but the teacher title structure is not perfect, the grass-roots teachers are less; students’evaluation of teaching level is higher, but the general medical basic theory ability is insufficient; general practitioner training bases are enough, but there are also some problems to be solved in no time.3. From arrange the learning time, propaganda general practitioners, improve students, teachers and training units’understanding of the importance of job-transfer training; the government must release the general practitioners training supporting policies; increase financial investment and to strengthen the hardware construction of training base to promote the general practitioners job-transfer training gradually standardized.Measures and suggestions:The first is the government should attach great importance to general practitioner training work; second is to strengthen training, select standard training object; third is to extend appropriately the time according to the need of training; training content should be practical, focus on medical theory and clinical skills; enrich training materials, perfect 《Examination Handbook》; making full use of remote education, teaching methods should be diversified; taking measures simultaneously to strengthen the management of teaching process; improve the quality of teachers, increase the number of grass root base teachers, improve teaching level and teaching ability of teachers; the fourth is to further strengthen the construction of training base GPs; the fifth is to increase the intensity of investment; the sixth is to make the training work of normality.
