Research On The Multilevel Training Program For ICU Nurses Based On Nursing Clinical Ladder Program

Objective:By surveying the status of the ICU nurse training demands for core competence, to investigate the influence factors and the highest training demands for nurses of different levels. Via the expert-consulting method, to build a multilevel training program suiting the ICU nurses in China, and to give a basis for building the multilevel training program in different places in China.Method:This study consists of two parts. In the first part, using self-made questionnaire forms to survey among133ICU nurses. Using the single factor analysis of variance and multiple regression analysis to analyze the influence factors, and to explore the highest training demands for nurses of different levels. In the second part, based on the literature review and field investigation, to build a preliminary multilevel training program for ICU nurses, including admission criterions with alternatives, multilevel training content, training targets, and so on. According to the research content,16specialists in the fields of ICU medical treatment, ICU nursing, nurse management, and nurse education were chosen. After two times expert-consulting, we gathered and analyzed the suggestions given by specialists and consulting results, so as to build a multilevel training program for ICU nurses.Results:①The survey for the status of ICU nurses results:The training demands for core competence of ICU nurses are in a relatively high level (4.39±0.72). The requirement of "mastering and applying the ICU professional technology capability" is the highest training demand. The single factor analysis and multiple regression analysis shows that, ICU working experience, the highest education background, self-evaluation job competency and the training will affect the training demand for core competence. Nurses of different levels need different dimensions and targets.②The expert-consulting result:After two times expert-consulting, a multilevel training program for ICU nurses was built, including the admission criterions with alternatives, multilevel training content, training targets, admission training levels of the training targets, and raising schematic diagram of the ICU nurse multilevel core competence training. The training targets contain5one level targets,27two level target, and252three level targets. The training program shows the training focus for nurses in different levels, indicates the ICU nurse improving process, and can give an example for the ICU nurse multilevel core competence training in other hospitals.Conclusion①The training demands for core competence of ICU nurses are in a relatively high level. Consequently, we should pay much attention to the ICU nurse core competence training, focusing on ICU techniques training and nurse self-development, and considering different levels, ability and training will, so as to perform a systematic, comprehensive, people-oriented, targeted, multi-level training mode. We should reduce nurse pressure, improve training effect, make sure critical nursing quality and security, and promote nursing development.②The multilevel training program for ICU nurses we built includes the admission criterions with alternatives, training content of each level, three-level training targets, admission training levels of the training targets, and raising schematic diagram of the ICU nurse multilevel core competence training, which provides a basis for performing ICU nurse multilevel training, promoting ICU nurses, improving ICU nursing quality, and developing nursing cause.
