Background Currently, the quality of life in patients with terminal cancer research focuses on the treatment of patients hospitalized for drug treatment, treatment methods, and other aspects of nursing intervention for terminal cancer patients quality of life. Because the lack of specific symptoms of early cancer, most cancer patients are diagnosed when already terminal, the prognosis is poor. In recent years, will introduce the concept of hospice patients with terminal cancer in clinical practice, to provide holistic care for the terminally ill, become a national medical institutions respected and effective means.Objective By investigating the quality of life in patients with terminal cancer who were accepted hospice services, and evaluating its impact factors, to provide a reference for developing nursing interventions with terminal cancer patients.Methods Household survey of270cases of poor patients with advanced cancer who were meet hospices receive standard and accepted hospice services of the Third Affiliated Hospital of Xinxiang Medical University from December2011to November2012, with a questionnaire composed of normal information questionnaire, EORTC QLQ-C30version3.0and KPS score sheet. All patients completed QLQ questionnaires before and one month after receiving hospice service. The mean and standard deviation was used statistical description of measurement data. A paired t-test was used to analyze the score of each dimension of the quality of life of patients with terminal cancer who were received hospice services before and after, T-test and F-test were conducted to assess the impact factors of the life quality on patients with terminal cancer that accepted hospice services, and a stepwise regression analysis was conducted to determine the influencing factors on global QOL as well as each dimension.Results1.Totally270patients with terminal cancer received hospice services were investigated in this study. Types of cancer:81cases of lung cancer,39cases of stomach cancer,33cases of colorectal cancer,30cases of liver cancer,24cases of esophageal cancer,17cases of cervical cancer,13cases of breast cancer,11cases of prostate cancer,9cases of nasopharyngeal carcinoma,7cases of laryngeal cancer,2cases of thyroid cancer,2cases of lymphoma, bladder cancer and osteosarcoma in1case. The average score of the global QOL for patients before and after hospice services were45.76±23.28, and56.42±20.05, it is showed remarkable improvement of life quality after the hospice services(P<0.05).2. According to the results, gender, age, education level, marital status, and informed the extent of the disease are main influencing factors on the QOL of patients with terminal cancer receiving hospice services. Marital status, and informed the extent of the disease have significant effects on the global QOL, patients with spouse had a higher quality of life than patients without spouse (unmarried/divorced/widowed); The higher the extent of the disease, the worse the overall quality of life. Gender and age have significant effects on the physical functioning, male patients feel better than females in physical functioning, and physical function is getting worse with age. marital status, and informed the extent of the disease have significant effects on the emotional functioning, patients with spouse had a better emotional functioning than patients without spouse, the lower the extent of the disease, the better the emotional functioning. Age and marital status have significant effects on the role function, the role function is getting worse with age, and patients with spouse had a better role functioning than patients without spouse. Age and education level have significant effects on the cognitive function, the cognitive function is getting worse with age, the higher the education level, the better the cognitive function. Informed the extent of the disease and education level have significant effects on the social function, the lower the extent of the disease and the higher the education level, the better the social functioning.3. Results of Pearson correlation show that, scores of the global QOL, physical function and emotional functioning were positively correlated with KPS score. In addition, scores of fatigue, pain, loss of appetite, vomiting, sleep disorders and diarrheas were negatively correlated with KPS score.Conclusions1. The quality of the patients with advanced cancer accepted hospice services than before receiving hospice services significantly improved, especially in the patient’s emotional functioning, pain, symptom improvement.2. Gender, age, education level, marital status, and informed the extent of the disease are main influencing factors on the QOL of patients with terminal cancer receiving hospice services.3. Scores of functional areas were positively correlated with KPS score, and scores of symptoms were negatively correlated with KPS score.
