[objeetive]1.To understand the health education needs for total gastrectomy patients after hospital2.Seek and formulate a continuity health education mode that is suitable for the patients after total gastrectomy, and improve the quality of life through health education[Methods]The patients receiving the total gastrectomy and Roux-en-Y anastomosis which elected from Tianjin Medical University hospital and Cancer Hospital. The inclusion was:the first time by clinical, pathological diagnosis of gastric cancer; accept voluntary health education, survey and laboratory testing; generally in good condition in the two groups of patients before surgery, KPS score greater than or equal to90points, the expected survival was more than3months; junior high school and junior high school education; no preoperative neoadjuvant chemotherapy; recovered well after surgery, discharged within2weeks after surgery, no complications such as gastrointestinal fistula. Excluded was:participation in other clinical trials, distant metastasis or suffering from other serious somatic, mental illness, severe mental or cognitive disabilities.All patients discharged from hospital accepted the conventional discharge guidance, observation group was given the continuity of health education intervention group after discharge, while the control group did not receive the health education. With the help of a full-time nurse To help the patients in observation group to develop the right pattern of diet, maintain the good mental state and live the regular pattern, patients were received the health education by personal exchanges, seminars, telephone, internet and videos, while the control group did not receive any education. In addition, the full-time nurses and patients agree that the review time, help up appointment as directed and receive a single laboratory.The appraisal of health status was following the QLQ-C30quality of life survey. The survey includes five functional scales (physical function, role function, cognitive function, emotional function and social functioning), three symptom scales (fatigue, nausea and vomiting, pain), six individual measurement items and an overall quality of life, In the section of QLQ-C30,the higher the overall health status score, the better the higher the quality of life; the higher functional scale scores, indicated the higher the quality of life; while the higher the symptom scale means the quality of life was worse. All patients accepted the quality of life questionnaire and measurement of body weight, laboratory hemoglobin, albumin and total protein after one month and three-month.[Results]There was no statistical difference between observation group and control group in health status after one month. While after health education in observation group, patients scored more in the quality of life survey than control group in three months after surgery. Observation group had better physical function, role function, emotional function field than control group. And there was significantly different in the aspects of fatigue, nausea, vomiting, insomnia and other symptoms.[Conelusions]The continuity of health education after discharge can improve the quality of life in patients of gastric cancer, it can solve the practical problems, it was worthy of promotion.
