Objective:Explore the role of comprehensive nursing intervention on falls prevention community elderlyhypertensive patients with diabetes,Provide a reliable basis for the further implementation of fallsintervention.Methods:Established residents of the community health service stations in Shihezi City Health Records,The typical sampling technique was used to select hypertension and diabetes incidence5,21,42district asthe sample communities,100patients met the inclusion criteria, extracted from the sample communities asresearch subjects. Follow a random principle subjects were divided into intervention group and the controlgroup,50in each study. group home visits once a month, every home visits about30to50minutes, a totalof six comprehensive nursing intervention, Intervention measures included health education,dietaryguidance,medication guide,Athletic Director, balance training guidance, home environment, guidance anddaily life guidance. The control group did not implement any intervention, Issuance of fall-relatedknowledge health education materials at the end of the study. Use fall rate of cognitive knowledge,take fallprevention measures rate, the transformation rate and the falling rate of home environment evaluationcomparison intervene.Results:①Fall rates:Before the comprehensive nursing intervention,Falls to the control group was18.0%,24.0%in the intervention group, The two sets of fall rates compared, The difference was notstatistically significant(x~2=0.542,P>0.05). After comprehensive nursing intervention, The fall rate was26.0%in the control group,the intervention group was8.0%, The two sets of fall rates compared, Thedifference was statistically significant(x~2=5.741,P<0.05).②Fall rate of cognitive: Before thecomprehensive nursing intervention, Comparison of two groups of patients fall risk factors for cognitive,The difference was not statistically significant(P>0.05). After comprehensive nursing intervention,Intervention group of elderly patients on fall risk factors for osteoporosis or calcium deficiency(x~2=6.618,P=0.010), Taking antihypertensive hypoglycemic agents (x~2=10.866, P=0.001), Takingsedative-hypnotics(x~2=4.167,P=0.041)And other cognitive rate was higher, The difference wasstatistically significant (P <0.05).③Take measures to prevent a fall rates: Before the comprehensivenursing intervention, Comparison of two groups of patients fall prevention behaviors, The difference wasnot statistically significant(P>0.05). After comprehensive nursing intervention, Intervention group ofelderly patients with reasonable dietary supplement (x~2=3.934,P=0.047),calcium and vitamins(x~2=13.000,P<0.001), and other measures to prevent falls proportion higher than that in the controlgroup, The difference was statistically significant (P <0.05).④Before the comprehensive nursingintervention, The home environment of the two groups of patients fall risk factors to compare, Thedifference was not statistically significant (P>0.05). After comprehensive nursing intervention,Intervention group patients home environment falls risk factors for furniture solid (x~2=6.857,P=0.009),door rugs are not fixed(x~2=34.167,P=0.009), the bathroom is not laying anti-skid pad(x~2=6.353,P=0.012) significantly less than that of the control group, The difference was statistically significant (P <0.05). Community elderly patients home environment transformation projects included Clean up debris(60.0%), Fixed door and small and medium carpet(52.6%), Secure furniture(52.5%) and so on.Unmodified project included Furniture edge or the corner is not smooth and outstanding at right angles, thestairs not installed a two-way armrest and so on.Conclusion:Comprehensive nursing intervention can effectively improve the rate of cognitive communityelderly patients fall, Prompted to take measures to prevent falls events, Reduce fall rates,Communitymedical personnel should actively carry out comprehensive nursing intervention,In order to improve thecommunity elderly patients quality of life.
