BackgroundDiscogenic low back pain in the elderly is a common and frequently-occurring disease. Injury, degeneration of lumbar intervertebral disc and local inflammation is the main pathological basis. In recent years, studies have shown close relationship between spinal static and dynamic balance:spinal static instability usually are accompanied wi th dysfunction of paraspinal muscles, such as psoas major. Clinically, discogenic lumbago and abdominal pain are called "lumbago due to Dai Vessel stasis". Treatment by adjusting the paraspinal muscle may improve lumbar function, reduce pain. Under the guidance of this theory, we treat discogenic lumbago with psoas major strumming, and achieved good clinical result. Whether paraspinal muscles strumming can make impact on pathological changes of intervertebral disc injury, need some basic research to answer.ObjectiveTo explain the mechanism of psoas muscle strumming treatment to discogenic lumbago, and to provide experimental evidence for the new technique and theory of lumbago due to stasis of Dai Vessel. Established the lumbar intervertebral disc injury model to simulate the pathological process of discogenic lumbago. Treat the model with strumming psoas muscle, and observed the changes in animal behavior, disc morphology and immunohistochemistry level of IL-1β, collagen type Ⅱ in the disc.MethodsWistar male rats (n=36) were randomly divided into the intervertebral disc surgery group (n=24) and sham surgery group (n=12). Anterior approach to lumbar spine surgery was performed, punctured the anterior annulus fibrosus in intervertebral disc surgery group rather than the sham surgery group. And then disc surgery animals were divided further into surgery model group (n=12) and strumming treatment group (n=12). Two weeks after the surgery, strumming treatment group were intervened with strumming psoas muscle in prone position, every other day for10times (3weeks). At the points of2weeks and5weeks after surgery, all animals were detected to show the intensity of pain by the standing-up activities in open field testing and the thermal paw withdrawal latency in paw thermal stimulator system. After behavioral test, the animals were sacrificed, completely colleted the punctured discs for HE staining and immunohistochemistry study of IL-1β and collagen typo Ⅱ. At last statistical method was used to analysis datasResults1. Behavioral detection:1.1Open-field behavior detection: Two weeks after surgery modeling, compared with the sham surgery group, standing-up activities of the disc surgery model group decreased significantly (P<0.05). Five weeks after surgery modeling, compared with the sham group, standing-up activities of the disc surgery model group decreased significantly (P<0.05); and compared with the disc surgery model group, standing-up activities of the strumming treatment group increased significantly (P<0.05).1.2Thermal paw withdrawal latency detection:Two weeks after surgery modeling, compared with the sham surgery group, thermal paw withdrawal latency of the disc surgery model group decreased (P<0.05); but five weeks after surgery modeling, there is no statistical difference between groups.2. Disc HE staining:Nucleus pulposus:for sham surgery group, nucleus pulposus is denser, evenly distributed in central area; for disc surgery model group, nucleus pulposus is sparse, uneven distributed, and the number of cells was significantly reduced; for strumming treatment group, the nucleus pulposus is better than disc surgery model group.Annulus fibrosus:for sham surgery group, annulus fibrosus arranged in concentric circles regularly; for disc surgery model group, arrangement of tissues of annulus fibrosus is not well structured; for strumming treatment group, the annulus fibrosus is better than disc surgery model group.3. Immunohistochemisty results:Disc IL-1β immunohistochemical expression mainly in the annulus cells. Compared with sham surgery group, positive expression of the disc surgery model group increased significantly (P<0.05); and compared with the disc surgery model group, positive expression of strumming treatment group decreased significantly (P<0.05).Nucleus pulposus collagen type II immunohistochemistry:Compared with the sham surgery group, positive expression of the disc surgery model group decreased significantly (P<0.05); Compared with the disc surgery model group, positive expression of the strumming treatment group increased significantly (P<0.05).Conclusions1. Results of standing-up behavior, disc HE staining and immunochemistrical expression of IL-1β and collagen type Ⅱ show that, the dise puncture injury rat model is a good model for discogenic lumbago.2. Results of standing-up behavior, changes of intervcrtebral disc morphology and immunochemistrical expression of IL-1β and collagen type II show that, strumming psoas muscle treatment can promote the recovery of the disc injury rat model.3. Technique of strumming psoas muscle, adjusting the spine dynamic structure, may make a good impact on the static structure like intervertebral disc.
