Applied Anatomy And Clinical Significance Of Genitofemoral Nerve(GFN) In Psoas Major Muscle

Object: To realize the morphologic feature of GFN in psoas major muscle by cadaveric dissections,the study provided the anatomical basis for lumbar anterior approaches. Methods: To select 15 embalmed man cadavers,split psoas major muscle carefully ,find the branches of the GFN were traced to their sites of innervation and their morphologic features. Select the bony or evetion mark as labeling,to measure data,then carry out statistical treatment. Result: GFN is divided into three patterns in psoa(?) major muscle,Type Ⅰ :GFN is to sent out from L1 nerve root alone and descends obliquely forward through the psoas major muscle, piercing the anterior fascia on the psoas major muscle above the L3-4 disc,to occupy about 20%(6/30).Type Ⅱ: GFN is to sent out from L1 and L2 nerve roots which they become intermingled and descends obliquely forward through the psoas major muscle, piercing the anterior fascia on the psoas major muscle below the L3-4 disc mostly,to occupy about 56.7%(17/30).Type Ⅲ: GFN is to sent out from L1 and L2 nerve roots and divided into genital branch and femoral branch . Genital branch descends obliquely forward through the psoas major muscle, piercing the anterior fascia on the psoas major muscle above or below the L3-4 disc. Femoral branch passes through the psoas major muscle medially mostly, to occupy about 23.3%(7/30).GFN's length inner psoas major muscle is about 4.850±0.788cm on the left and 4.533±0.851cm on the right ,p>0.05, Conclusions:GFN will be injured when expose for lumbar anterior approaches from L2 to L4.
