Research In Clinical Application About TuiNa-Acupuncture Therapeutic For Osteoporosis Lumbago Of Kidney Deficiency Type

ObjectiveObserve and compare the effect of TuiNa-Acupuncture and calcium carbonate(Caltrate D600 Tablets) on lumbago in kidney deficiency type, in order to provide safe and effective Tuina-acupuncture method for Osteoporosis in clinical application。Method64 patients were randomly allocated to observation and control group,34 cases in observation group and 30 cases in control group. The observation group was treated by TuiNa-Acupuncture and the control group by oral administration of Caltrate D600 Tablets.The observation group applies TuiNa methods on the lumbar, such as rolling, pushing, pressing, grasping, shaking etc. At the same time acupuncture on Kid.3 and UB.40 in both legs. One treatment last 30 minutes and 10 times is one course. Totally treat 30 times, it's around 3 months. The control group takes oral administration of vitamin D calcium tablets (Caltrate D600 Tablets),600mg. per day. It takes 3 months, too. After 3 months, compare the blood Ca, P, ALT and by measurement of bone metabolism markers BGP and DPD with observation and control groups. Biomechanical properties of bone could provide important and direct reference for osteoporosis, so measure the data from lumbar CR pictures, like lumbar curvature K, lumbar lord tic index, sacrum slant angle, lumbar sacral angle, the fifth lumbar vertebral index, the third lumbar vertebral index. And then evaluation visual analogue scale(VAS), the differentiation of VAS between it before treatment and after first treatment, first months, second months and third months, used to evaluate the treatment effect.Data Process and Statistical Analysis use MS Excel2003 and SPSS 13.0, Quantitative data in normal distribution was processed by analysis of variance, while in abnormal distribution, it was analyzed by Wilcoxon test.ResultsThe result of Ca, P and ALP:There were no significant differences of Ca and ALP after the treatment between the groups(P>0.05), but P displays significant differences, P<0.05 in the control group and P<0.01 in the observation group.The result of BGP and DPD:Comparison of BGP and DPD with same group between after the treatment and before treatment, it shows significant differences in statistics (P<0.05), even more effectiveness of the control group (P<0.01). But it's not meaning that between the different groups have significant differences of effect after treatment(P>0.05).The result of biomechanical properties of lumbar vertebra by CR:Measure the 6 groups data from lumbar CR photo, like lumbar curvature K, lumbar lord tic index, sacrum slant angle, lumbar sacral angle, the fifth lumbar vertebral index, the third lumbar vertebral index. The study shows in the control group, lumbar lord tic index and sacrum slant angle, have significant differences in statistics between after the treatment and before treatment (P<0.05), inside P<0.01 of lumbar lord tic index. Besides above, comparison of lumbar lord tic index in observation and control group have significant differences, the control group is better than the observation group (P<0.05). And then in observation group, there is significant differences after taking Caltrate D600 Tablets (P<0.05). The other index aren't significant differences in the group or between the group(all P>0.05).The result of VAS:Comparison VAS scores in the observation and control groups about osteoporosis lumbago in kidney deficiency type, at before treatment, after first treatment,1 month,2 month and 3 month, totally 5 time points to measure the VAS scores. Both groups VAS scores changing trends is similar, from slow up and till 2 to 3 month suddenly down. At beginning 3 time points of all course, it's no significant differences between two groups(P>0.05). But last 2 time points of the course(2 month and 3 month after treatment), VAS scores express significant differences, VAS scores in the control group is different from the observation group's in statistics,2 month time point is P<0.05 and 3 month time point is P<0.01.Conclusion1. TuiNa-Acupuncture and oral administration of Caltrate D600 Tablets are difficult to promote blood Ca+ and ALP in 3 months. From study, maybe P- changed by treatment in early stage of osteoporosis.2. By measurement of bone metabolism markers BGP and DPD, means TuiNa-Acupuncture method is one of the effective treatments to prevent and treat osteoporosis in early stage.3. TuiNa-Acupuncture provide a good way to improve biomechanical properties of lumbar vertebra, relief pain in osteoporosis of kidney deficiency type.4. TuiNa-Acupuncture is better method to reduce pain than oral administration of Caltrate D600, base on the study the new method of TuiNa-Acupuncture will be effect, safe, simple and economic treatment in clinic.Above all, TuiNa-Acupuncture, the new method for osteoporosis promotion need more cases and more time to confirm.
