Analysis Of Clinical Data For1419Breast Cancer And Prognostic Factor For Breast Conservative Surgery

Objective:To investigate the clinicopathological significance in breast cancer patients underwent surgical treatment and to find out possible prognostic factors in patients underwent breast conservative surgery.Methods:We studied the clinical features in a cohort of1419patients with breast cancer, who underwent surgical treatment from January2004to December2011at Zhongshan hospital Fudan university. We also analyzed the possible prognostic factors in those who underwent breast conservative surgery.Results:1. The median age was53years old(range,22-95years).77%of patients were diagnosed with infiltrating ductal carcinoma.Among those patients, Stage I cases was found in32.1%and Stage â…¡ cases was found in31.5%. There were46.4%patients with LuminalA,19.9%patients with LuminalB,5.8%patients with HER2-positive type,18.7%patients with triple-negative type and9.2%unknown.2. With a median follow-up of36months, there were no differences in disease free survival rate and overall survival rate in patients with early-stage of breast cancer treated with either breast conservative surgery or mastectomy. Young patients with early-stage of breast cancer, received breast conservative surgery or mastectomy, confirmed no difference in disease free survival rate and overall survival rate.3. Univariate analysis suggested that tumor size, axillary lymph nodal status, pTNM stage and cancer embolus in lymph vessel and blood vessel are prognostic factors of breast conservative surgery. Patients with HER2positive or young age have higher risk of local recurrence after breast conservative surgery.Conclusions:Patients with Early-stage of breast cancer have similar survival rates treated with either breast conservative surgery or mastectomy. Tumor size, axillary lymph nodal status, pTNM stage and cancer embolus in lymph and blood vessel are prognostic factors of breast conservative surgery. Concerning to the clinical and pathological features of breast cancer, breast concervative surgery is safe for patients with early-stage of breast cancer. Besides, it provides better quality of life, breast conservative surgery can be a good choice.
